EE STARTED THE DAY AND HAD WORKED AT THE SHOP LOCATED IN LEHI, UTAH FOR THE MAJORITY OF EE'S WHOLE SHIFT. THERE WERE MATERIALS THAT NEEDED TO BE TAKEN TO OUR CREW TO CONTINUE WORKING. EE DROVE TO THE WORKSITE. WHILE EE WAS THERE, EE WAS WALKING AND WHILE WALKING EE WAS STRUCK BY THE FORKS OF A PEICE OF EQUIPMENT THAT FELL OVER, WHICH RESULTED IN EE'S LIFE ENDING.

A Komatsu 605HD rolled over backwards at the lower crushing material dump site, landing upside down. The wall was approximately 25 feet high. Employee was wearing a seatbelt at the time of the roll over.

Employee was driving a haul truck and was being loaded when that caused EE to be jarred in the cab of the truck causing muscle spasms in EE's back. This occurred on 11/22, went to the Dr. on 11/23 and treated conservatively until yesterday. The Dr. hasn't seen any improvement so Dr. ordered Physical Therapy and prescriptions making the recordable date 11/29/22.

Entering loader, reached for the handrail on the door while stepping to top stair. Missed door handrail & fell backwards. Caught self with hand. This action caused a heavy load on EE's right shoulder. MRI shows a Bicep tear. Delay in reporting as Insurance has been debating if the tears are Chronic or Acute.

Employee was working on wash screen and felt discomfort in eye. EE thought some dirt was in it from sweat and was wearing safety glasses. EE used the wash station at work and went home at 2:30 am. At 8 am that same morning EE still felt discomfort and called EE's supervisor for treatment. EE was treated for foreign debris in eye.

EE woke with severe chest pain at approx. 3:30 a.m. and believed it to be really bad heart burn, he got ready for work and then drove to the office. Upon arrival, he started experiencing more of the pain with some tingling in leg and arm. When another EE arrived he asked her to call 911. EE was coherent and communicating with EMT's at departure from the mine.

EMPLOYEE WAS LUBRICATING A BELT ON THE FORKLIFT IN PREPERATION TO REMOVE THE BELT. ANOTHER EMPLOYEE OPERATING A BOBCAT WAS PULLING ON THE FORKLIFT, THE BOBCAT'S WHEELS BEGAN TO SPIN SO THE OPERATOR TURNED IT OFF. THIS CAUSED THE FORK OF THE LIFT TO DROP ONTO THE MECHANIC'S HAND.
