EE was hauling dirt. While in the yard he noticed a large rock between two tires on his trailer. He grabbed his hammer and started hitting the rock to get it out of there. Hit it a couple of times and then both tires blew while he was hitting at the rock. After the tires blew, the loader operator looked and noticed him on the ground. He called his manager to call 911.

Employee was assisting in the repair & replacement of the hub & flights on the rock scrubber at the wet plant. Equipment was locked out & tagged out. Two employees were lifting the hub into the rock scrubber with a loader & were removing the hub into the scrubber. Hub was heavier than expected & his finger was pinched between the hub & frame of scrubber causing a laceration.

EE WAS REMOVING FLIGHT FROM BLADEMILL, APPLYING PRESSURE WHEN IT CAME LOOSE AND FELL AGAINST SIDE OF BLADEMILL. EE RECEIVED LACERATION TO HIS FINGER WHICH REQUIRED 6 STITCHES.
EMPLOYEE WAS WELDING INSIDE CHUTE AT WASH PLANT WHEN A PIECE OF SLAG & WELD FELL ON HIS KNEE AND BURNT THROUGH HIS PANTS AND ROLLED INTO HIS BOOT. BURNT INTO TOP OF FOOT BEFORE HE COULD GET BO OT OFF. THIS HAPPENED ON FRIDAY. HE DID NOT GO TO DOCTOR UNTIL SUNDAY WHEN FOOT GOT INFECTED.

WASH PLANT OPERATOR WAS INSTALLING RUBBER HOSE ON PIPE FITTING. WHILE CUTTING HOSE WITH UTILITY KNIFE, KNIFE SLIPPED THROUGH RUBBER AND CUT HIS LEFT HAND BETWEEN THUMB AND FIRST FINGER. HE WAS SENT TO CONCENTRA AND RECEIVED STITCHES. HE RETURNED TO WORK NEXT SHIFT.
