The employee sought medical treatment for feeling ill and was misdiagnosed, received treatment, and released from care. The employee returned to work for two days and felt unwell again, returning to the ER on 3/12/25. Further testing confirmed a different illness on 3/16/25.

Showing all 6Employee injured his left leg calf while walking up an inclined catwalk adjacent to the dry sand belt at the Rock Plant. As he was walking, he felt a "pop" in his left leg. He reported pain 10 minutes later and was taken to Simi Health Center Urgent Care and received first aid treatment.
A 65-year old man was found dead in his automobile in a remote area of the mine site. It appears the deceased had strayed onto the mine site after he left his job on an adjacent property.
Employee was walking up the stairs at the Specialty Plant. He felt his knee pop. Employee did not report this until April 9, 2007.
Continuing back pain.
