This was a medical occurrence only. Customer truck driver was backed into stockpile waiting to get loaded when EE started having chest pains. Called 911 they took to Pulaski hospital where they found EE's blood pressure was elevated.

This is not an accident or injury. Employee was operating a loader when EE needed to use the restroom, EE went to stand and could not feel EE's leg, ambulance took to hospital where they found EE had low potassium, EE was released and told to return to work Monday morning.

Employee operating a excavator, throwing material off bench to pit below. Employee called Foreman asked him to come to excavator. Foreman arrived employee stated he did not feel well & left arm hurting. They immediately put employee in vehicle took to hospital. In route notified me I called 800# to report. Employee required 2 stents and will have open heart for one blockage.

EE WAS DRILLING, HE WENT TO CHANGE DRILL STEEL, HE STATES THAT A GUST OF WIND CAUGHT THE STEEL CAUSING IT TO START TO FALL, EE GRABBED IT STRAINING HIS BACK BUT LATER REPORTED THAT HIS SHOULDE R WAS HURTING.

EE WAS DRILLING ON TOP OF HILL. IT WAS A WINDY DYA, WHEN A GUST OF WIND BLEW SOME DUST INTO EE'S EYES UP UNDER HIS SAFETY GLASSES, CAUSING IRRITATION TO EYES.
