It was raining, ground wet, employee was walking into mud room at sand plant when they slipped in mud and polymer causing them to fall. States they have back and knee pain.

Employee was greasing the loader first thing in morning when EE slipped and fell.

This is not an accident or injury. Employee was operating a loader when EE needed to use the restroom, EE went to stand and could not feel EE's leg, ambulance took to hospital where they found EE had low potassium, EE was released and told to return to work Monday morning.

Employee was opening our entrance gates, the gate has a wheel on it to where you roll gate open, wheel got turned employee kicked wheel with foot when EE heard and felt a pop in knee causing a strain to knee area.

Employee was exiting loader, when ee reached the 3rd rung hands slipped on handrails causing ee to fall to ground. At the time it was raining hard and handrails were wet. Took employee to ER to get checked out and they found nothing other then some bruising. Dr's excuse said to return in 2 days but employee was not scheduled to work except for half a day the following day.

Employee was removing a metal strainer from a pvc line at our Sand plant, when he tried to remove the pvc line from the metal strainer he cut top of left thumb.

Employee was standing on crusher platform using a digging bar to try and dislodge a wedged rock in jaw crusher feeder when bar slipped catching his finger between the bar and handrail. This caused a laceration to tip of ring finger on right hand requiring 8 stitches.

EE was on a scaffold 4' off ground welding hooks to hold electrical cables, he went to move and a board had been moved causing him to fall. Lanyard caught him but he tit his rt hand on scaffold causing injury. He did not report to management. Co-workers asked if he needed to go to Dr, he stated he was okay. Reported 11/12/12. He has surgery for joint locked in flexion.

Employee was walking to #1 tailpulley when he stepped across about a 2 foot puddle of water, he felt a pop in his leg. Employee pulled a muscle in upper right leg, causing him to use crutches with limited walking.

EE was putting new brake pads on dump truck when he felt a strain in lower back. Employee went to doctor and was put on light duty work for 4 days.We are looking into this accident. We had a helper with him, he did not report any injury friday before he left work but came in monday morning first thing and reported back injury.

Employee was helping unload motor, motor slipped, caught rt thumb between motor and truck bed, causing brusing to rt thumb. Doctor put rt hand in a brace.

Employee was using a sledge hammer to put side rail on screen; went to hit side rail, handle of hammer caught causing him to miss the intended target and strik his right index finger.

Employee was on platform below jaw crusher when his foot slipped causing him to fall backwards on a rock causing bruising to lower back.

EMPLOYEE WAS WELDING & CUTTING ON GUARD AT PLANT. PIECE OF METAL SHAVINGS WENT UNDER SAFETY GLASSES INTO RIGHT EYE CAUSING IRRITATION. WENT TO DOCTOR WHERE METALLIC FOREIGN BODY WAS REMOVED.

EMPLOYEE WAS UNDER DUMP TRUCK ADJUSTING CLUTCH WHEN DIRT AND METAL SHAVINGS FELL IN AROUND EMPLOYEE'S SAFETY GLASSES INTO EYES. EMPLOYEE WENT TO DOCTOR WHERE 3 SMALL PIECES OF METAL WERE REMOVED FROM EYE.

EE WAS CUTTING METAL OFF A HANDRAIL, THE BLAST OF METAL WENT UNDER HIS CUTTING GLASSES AND A PIECE OF METAL WENT INTO EYE.

EE WAS HELPING TO PUT SCREENS IN, EDGES OF SCREEN NEEDED TO BE TRIMMED OFF, WHILE USING A KNIFE TO CUT EDGES OFF, KNIFE SLIPPED AND CUT FINGER.

EE WAS DRILLING, HE WENT TO CHANGE DRILL STEEL, HE STATES THAT A GUST OF WIND CAUGHT THE STEEL CAUSING IT TO START TO FALL, EE GRABBED IT STRAINING HIS BACK BUT LATER REPORTED THAT HIS SHOULDE R WAS HURTING.

DOING REPAIR WORK ON OUR CASE SKID LOADER. EE WAS USING A BAR TO TRY AND UNLATCH BUCKET, THE BAR SLIPPED AND EE FELL FORWARD STRAINING HIS BACK.

EE WAS STOCKPILING. AFTER HE HAD DUMPED HIS LOAD OF ROCK HE COULDN'T GET HIS TAILGATE CLOSED, HE WENT TO THE BACK OF THE TRUCK AND SAW ROCK BETWEEN THE BED OF THE TRUCK AND TAILGATE, HE LIFTED UP TAILGATE TO LET ROCK OUT, WHEN HE DID TAILGATE CAUGHT HIS LEFT MIDDLE FINGER AND BROKE IT.

EE WAS SHOVELING MATERIAL OUT FROM UNDER BINS FOR APPROX. 10 MINUTES WHEN HE FELT PAIN IN HIS LOWER BACK AND DOWN HIS LEG.

EE WAS DRILLING ON TOP OF HILL. IT WAS A WINDY DYA, WHEN A GUST OF WIND BLEW SOME DUST INTO EE'S EYES UP UNDER HIS SAFETY GLASSES, CAUSING IRRITATION TO EYES.
