Employee got into bed of service truck to ride to office. EE stated EE was sitting with EE's back to tailgate, tailgate popped open allowing EE to fall backwards onto ground, cutting back of head which required 2 stitches.

Injured was removing dirt off a R60 Hauler with Compressed air, piece of debris went under safety glasses and into their eye

This was a medical occurrence only. Customer truck driver was backed into stockpile waiting to get loaded when EE started having chest pains. Called 911 they took to Pulaski hospital where they found EE's blood pressure was elevated.

Was removing conveyor belt off welding truck, hit hand against vise, next day hand started hurting. Went to DR took x-ray, would let EE know. They called July 5 and said EE did have piece of metal. They removed it July 6, no stitches, no lost time.

Employee was carrying a chain over shoulder at secondary plant, ground was muddy, foot slipped causing the chain to swing around and hit employee in the mouth chipping a tooth.

Employee was putting teeth on a 988 Cat Loader. He was using a hammer to knock pin clips in when he hit the tooth with hammer causing a piece of metal to chip off and enter employee's upper leg. As of now its still not decided if they will remove or leave metal in.

Employee operates heavy equipment. He went to climb ladder on a R-60 Euclid Haul truck when he felt pain in his back

Employee had mud on his boots when he stepped on a piece of conveyor hood that was on ground and slipped landing on back and hip.

EMPLOYEE WAS WORKING ON 988F LOADER. EMPLOYEE CRAWLED BETWEEN TIRE AND BODY OF MACHINE TO WELD ON HYDRAULIC TANK BRACKET, (IN AN AWKWARD POSITION), WHEN HE NOTICED HIS LEFT HAND GOING NUMB. MR I SHOWED THAT THE EMPLOYEE HAS HERNIATED DISC. (PLEASE SEE ATTACHED LETTER, GIVING MORE DETAIL.)

EE WAS USING A BELT KNIFE TO CUT A SECTION OF BELT FOR SKIRT RUBBER, KNIFE BINDED INTO BELT BEING CUT, EE PULLED WITH FORCE, KNIFE CAME OUT AND STRUCK EE INHIS UPPER LEFT LEG, CAUSING A LACERA TION THAT REQUIRED STITCHES.

EE WAS CLEANING TOP OF PUNCH PLATES ON #1 SHAKER, AS HE HIT ROCK IT BUSTED IN SEVERAL PIECES AND CAME BACK AND HIT EE ON HIS RIGHT LEG AT THE SHIN AREA. EE DID NOT GO TO DR UNTIL 10-31-01.

HIT HIS HEAD ON LADDER OF TRUCK AND FELL OFF. HESLIPPED WHEN CLIMBING LADDER OF EQUIPMEN & FELL AGAINST TRUCK, HITTING HIS HEAD.

WHILE WALKING UNDER CRUSHING PLANT THE EMPLOYEE WAS HIT BY A ROCK THAT FELL OFF PLANT. THE ROCK HIT HIS CHEEK AND HE HAD SIX STITCHES PUT IN IT.
