EE had just finished loading trailer with the loader. As EE was preparing to leave there was a swarm of around 40 bats overhead and one flew directly for EE's face. EE swatted at the bat to keep it from hitting EE's face. Afterward EE noticed that EE's hand was scratched. The doc advised a full rabies vaccination series.

A dust bowl of lime blew into face penetrating safety goggles. Diagnosis - Acute Toxic Conjunctivitis of the right eye.

Operator attended safety meeting (6am) then performed inspection on loader. EE entered the cab and soon passed out in the cab (prior to operating). A haul truck operator secured them as EE was breathing but not responding. Ambulance came and EE was responding at that time. Earlier in the week the operator stayed home sick 2 days(dehydrated and they required IV when at hospital).

Emp was being fitted for custom earplugs. While removing plugs, vendor caused pain and injury to both ears. Emp was seen by Dr and returned to work. EE was later told to have surgery due to continued pain. This became a Reportable and Lost Time on 10-17-2017.

Employee was sitting in break room waiting for shift to begin. Employee started experiencing chest pains and not feeling well. Other employees coming into break room also noticed that he did not look well. Office was contacted blood pressure taken and first responders came up. Paramedics were called and employee was transported for further observation.

Secretary was working in the office late & when she went to lockup, as she opened the office door, the wind shipped the door open catching her purse on the door handle which then jerked her out the door. As she went to pull her purse loose she lost her balance & fell down the stairs striking the left side of her face on a stair or handrail. This resulted in a broken tooth.

SMALL PIECE OF METAL IN LEFT EYE. THE WIND WAS BLOWING DUST ALL DAY LONG, HAD DUST IN BOTH EYES ALL DAY LONG. WAS ALSO DOING CUTTING, GRINDING AND WELDING WITH STEEL FOR 4 HOURS. BUT I HAVE NO IDEA WHEN SMALL PIECE OF METAL ENETERED EYE.

A BAGHOUSE DISCHARGE PLUGGED & THE DUST WAS DUMPED OUT OF THE HOPPER ONTO THE GROUND. THE EEWAS SENT OVER TO VACUUM UP THE SPILL WITH A HIVAC INSTEAD OF STARTING AT EDGE OF PILE HE WALKED INTO IT GOING OVER TOP OF LOW TOP STEEL TOED SAFETY BOOTS DUST (LIME DUST) GOT INTO HIS BOOTS CHEMICAL BURN
