Employee of 6 months was working on crusher feeder motor the employee removed the guard, and there was more guarding around it so, they had to stretch. Employee was on their knees reaching arm-length away to diagnose and repair the motor. They stayed in that position for approximately 20 minutes. The pain increased gradually, it turned mild after they stood up.

EE reported to management on 10-12-21 that during their break they found a bump on their body. EE said they are not in pain but felt the bump and was concerned as they did not recall one there prior to 10-12-21. On 10-21-21 EE was diagnosed with an inguinal hernia.

Employee bent over to pick up an empty plastic bottle, 4 oz., that had dropped, and felt a sharp pain in the back. EE reported to the supervisor and went to an urgent care facility that took them out of work. Then EE went to preferred provider back specialist who said EE could work with restrictions and that the injury was not work related.

This is being reported as a non-employee injury. A driver for New England Pallet Company loosened a buckle on a strap and threw the strap over stack of pallets on a flatbed truck. A gust of wind blew the buckle and strap back toward non-employee hitting non-EE in the nose and eyes. Non-EE has a broken nose in two spots and one of non-EE eyes has a damaged lens.

Employee had entered the cab of a haul truck and was sitting in the seat. A wind gust caught the door as he was closing it and jerked his arm and shoulder.

Employee was found unconscious on a landing at the bottom of a set of stairs in the Screen Plant building. CPR was performed until EMS arrived. Employee was transported by ambulance to the hospital where he was later pronouced dead.

Employee was walking in the alley of the yard and a couple pieces of lime blew into his left eye. He was wearing the proper PPE of Safety Glasses with sideshields. He was treated at Advanced Eye Care where the Doctor removed the particles from his eye and gave him a prescription medication.

WHILE CHANGING INTO WORK CLOTHES IN THE LOCKER ROOM, HE LIFTED HIS LEG TO PUT HIS PANTS ON AND DEVELOPED SEVERE PAIN IN HIS BACK ON HIS LOWER RIGHT SIDE. IT RADIATES THROUGH HIS RIGHT BUTTOCK AND BEHIND HIS RIGHT LEG. (REPORTABLE DUE TO RESTRICTED WORK DAYS AND PRESCRIPTION DRUGS).

EMPLOYEE WAS REMOVING SPENT BRIQUETTES WITH A SHOVEL FROM THE CALCIUM FURNACES. HE STAGGERED AWAY AND COLLAPSED TO THE FLOOR IN A FELLOW EMPLOYEE'S ARMS. THE EMPLOYEE WAS UNRESPONSIVE SO TWO EMPLOYEES CONDUCTED CPR AND REVIVED HIM WITHIN A FEW MINUTES. HE WAS TAKEN TO A HOSPITAL VIA AMBULANCE AND ON 8/30/04 HAD HEART BYPASS SURGERY.

QUESTIONED EE ABOUT BRACE AND DISCOVERED HIS HAND WAS VERY SWOLLEN. SENT EE TO MD WHO TREATED EE FOR RT WRIST STRAIN. RELEASED EE TO MODIFIED DUTY WITH 15 LBS LIFTING, PUSHING, PULLING RESTRICTION TO RT ARM.
