While working at the Heavy Equipment staging area, employee received a bug bite on lower left side of chin. Not exactly sure when it happened. Received medical treatment on Sept. 14th and was released the same day for regular work activity.

While deploying liner material on the pond floor a gust of wind reached our work area. The gust lifted the liner material the IE was standing on the liner at the time the wind hit our location. The EE fell to the ground.

Employee was in break room when he noticed a burning sensation in left eye, had it washed at the dispensary at 10:00 am and again at 2:00 pm he then went to the clinic and had it checked again the clinic flushed it and sent him back to work with drops.

Bee sting on left arm, was building a new cat 785 haulage truck, under cab installing wiring harness. was in large open lot, went to move arm out of possiton, arm rubbed against reflective work vest. Bee on vest, stung worker on left arm. Allergic reaction set in.
