Employee got dust in their eye, scratched an eye and sought medical attention. Went to the eye doctor and was diagnosed with a conjunctival abrasion. There was nothing in the eye but was prescribed antibiotic eye drops.

The employee was loading trailers with another employee. EE told co-worker that didn't feel well. When EE was in the passenger seat of the tractor, EE passed out and fell over. EE's co-worker sought help from lime mine employees because EE couldn't move EE. EE was in cardiac arrest.

Employee reportedly lost consciousness while driving work truck and crashed into a concrete barrier. Employee wasn't injured from the wreck. A non-emergency ambulance was called to come to the site and check the employee out. The ambulance arrived at 1:30 pm and conducted some tests on the employee, employee's family took them to the hospital for further evaluation.

At approximately 3:40 PM, the employee contacted the foreman to tell the foreman the employee wasn't feeling well and had chest pains. The foreman brought the employee out of the mine and the Mine Superintendent transported the employee to the hospital. An examination and tests were performed at the hospital and the employee was released.

Miner complained of chest pains while driving truck through the portal on way to surface office. Forman contacted and ambulance called for care. Miner was transported to hospital. Returned to work next day OK, pains believed to be caused by acid reflux, pollen allergies and high blood pressure.

At approximately 510 PM there was a radio call that employee was having seizure. It was break time, employee was at the UG office. 911 was called. Foreman responded with the emergency truck and employee was brought out of the mine to surface mine office. The ambulance was directed to surface mine office. Employee was transported to hospital by ambulance.

Employee heard on radio gasping for air. Radios have name indictor on them. Employee found in seat of fuel truck struggling to breath and then became unresponsive. First response team found EE had no pulse. CPR and AED was used. Employee taken by ambulance to ER with cardiac arrest. After investigating this appears to be a medical emergency due to natural causes.

A fine mist of chemical released from bleeder port of pump at boiler system got carried in the air from the wind going through the building and went up into employees face and into his right eye.

Employee was shoveling lime under cooler belt, when a gust of wind blew, and he got lime in his eye.

While loading hydrated lime, employee turned the system off. However, the lime continued to flow after it was turned off. The control room became very dusty so he went outside the control room where conditions were very windy as he tried to get out of the dust. Employee had lime blow into his face receiving chemical burn to eyes and skin.
