Employee was greasing equipment, it was very hot this day, when he felt that his left foot felt very heavy. He sat down & drank some water & noticed that his left arm was going numb. Ambulance was called & taken to hospital. Nothing has been determined what results are yet. More testing will be done near future.

High winds - cleaning area of debris/barrels/equipment on North side of Vehicle Shop. Basket to skid loader was blown and tumbled into employee.
Employee was preparing to splice a conveyor belt, while kneeling he turned to the side and felt pain in his lower back. Was sent for medical attention on 2/22/11 place on restricted duty with physical therapy.
Employee turned from exiting preheater tower elevator when debris flew into his left eye. He was wearing safety glasses however wind was blowing at the time. Doctor removed debris and gave a prescription.
EE was cleaning the rocks and dust off the band. The work he was doing was above his head. He began to experience a hard pain in his chest. After break he worked on the 7th floor when he had same sharp pain, air shortage and felt his legs go numb. At this point he sat down. Employee asked him if he was OK & took him to 1st Aid. Ambulance called & he was taken to hospital.
Injured was operating quarry control panel when he felt a pain in his lower back. Injured was subsequently sent to company doctor on 2/24/10 and was put on restricted duty. No known conditions contributed illness.

While performing a walk through inspection of the pre-heater tower, non-metalic dust was blown into the right eye, by the wind. Safety glasses with side shields were being worn. The foriegn body was embedded in eye by rubbing. It was removed by a local physician, who issued a prescripition eye drop. There is no concern by the physician for permenant damage.

EE got foregin object ( dust particle) in left eye. Washed eye out went back to work , still continued to bother him on Monday 3/20/06. went to eye doctor remover foregin particle from EE eye. EE received an antibotic.
The injured employee was removing a section of plastic tubing from the Main Baghouse Inlet Pressure Tap. As the section of the pipe broke loose, the material on the pipe was disturbed. The employee felt material enter his left eye. The employee was wearing presciption safety glasses. Due to weather conditions on this day, wind gusts were common.
Employee was welding on 59 screw with proper protective equipment. Wind was blowing about 30 mph and caused unknown particles to get in right eye. Eye continued to bother employee and he was seen by an eye doctor on February 17 and received antibotic eye drops.

Fatal Heart Attack. Contract Truck Driver employed by D G Coleman Inc. (no MSHA Contr. ID). Driver was in Bulk Loading office area waiting for truck to be filled when he had heart attack. On-site First Aid Staff & paramedics tried to revive, 911 was called, transported to hospital where he was pronounced dead.
While sitting in passenger seat of water truck, the wind caught the open door slamming it on the left hand thumb. This caused laceration that required stitches. The work area is rather confined contributing to the injury.
On 6-5-04 the employee was squatting down to check a railcar when his left knee popped. No medical treatment was required. On 6-21-04 the employee squatted down to pick up trash when the same knee popped again. Ice was applied to the knee and he was sent to the company doctor.
EE WAS PERFORMING NORMAL REPAIR DUTIES. WINDY CONDITIONS CAUSED DUST TO BECOME AIRBORNE IN THE WORK AREA AND GET INTO HIS EYES. EE WAS WEARING SAFETY GLASSES.

UNKNOWN. FELT PAIN AND ITCHING ON LT. FOREARM. RAISED SHIRT TO DISCOVER RED SPOT. SUSPECTED INSECT BITE, NOT FROM A SPIDER (BROWN RECLUSE OR BLACK WIDOW).
WHILE WORKING IN AN AREA USED FOR WELDING PREPARATION EE FELT AN INSECT STING AND CONTINUED TO WORK THINKING NOTHING MORE OF IT. EE FINISHED HIS SHIFT AND THE NEXT. HE REPORTED THIS TO HIS SUP ERVISOR. HIS SUPERVISOR IMMEDIATELY SENT HIM TO THE DOCTOR. THE RESULTING STING SITE HAS GOTTEN BADLY INFECTED AND NOW REQUIRES FURTHER MEDICAL ATTENTION.
EE WAS WORKING ON THE DREDGE (WELDING) WHEN AN INSECT CRAWLED INTO HIS BOOT AND BIT HIM ON THE ANKLE. HE DID NOT SUFFER ANY PAIN UNTIL WORKING THE NEXT DAY AND AT 5:20 PM HE ALERTED MYSELF THE QUARRY MANAGER AND I IMMEDIATELY SENT HIM TO THE DOCTOR. HE WAS ABLE TO RETURN ON THE 7TH OF OCTOBER & DID RESUME FULL DUTIES. HE DID NOT RETURN THE NEXT SHIFT AS DR TOLD HIM NOT TO WEAR BOOT
EE WAS REMOVING BOLTS IN #2 COOLER & HAD STOPPED TO TAKE HIS SCHEDULED BREAK. HE STARTED EXPERIENCING CHEST PAIN & NUMBNESS IN HIS LEFT ARM. HE WAS TRANSPORTED TO THE HOSPITAL FOR EVALUATION. HE HAD 99% BLOCKAGE IN ONE ARTERY & A STINT WAS PUT IN PLACE.
SPIDER BITE - WORKING ON PRIMARY BELT.
PUTTING GUARD ON C11 CONVEYOR & WAS BITTEN.
EE WAS WORKING OUTSIDE IN VERY WINDY CONDITIONS.FOREIGN OBJECT BLEW INTO EYE. SUPERVISOR AND EE UNABLE TO DISLODGE OBJECT. THE NEXT DAY THE EE'SEYE WAS SWOLLEN AND RED. RX PRESCRIBED FOR CONJU NCTIVAL BURN.
OPERATOR HAD DRY MATERIAL IN THE SKID STEER BUCKET, WHEN OPERATOR DUMPED THE BUCKET WIND WAS BLOWING TOWARD HIM AND DUST GOT INTO HIS LEFT EYE. MEDICAL CLINIC PUT NO RESTRICTIONS. CLINIC CLEAN ED EYE AND USED ANTIBIOTICS.
REPAIRMAN WAS NEGOTIATING INGRESS AND EGRESS TO DUCTWORK TO COMPLETE REPAIR OF MATERIAL HANDLING SYSTEM. REPAIRMAN TWISTED HIS BACK DURING THE INRESS/EGRESS ACTIVITIES. EE CONTINUED WORKING TH RU WEEK. ON 7-7, EE VISITED HIS CHIROPRACTOR AS HIS BACK HURT. BY 7-13, EE WAS UNABLE TO WALK. 7-9- HE DID NOT REPORT FOR WORK.
CO-WORKER WAS TRAVELING ON THE ROADWAY & SAW #15 TRUCK SITTING IN A WATER POCKET, ABOUT 15' IN THE WATER, WEST OF THE ROADWAY. ANOTHER CO-WORKER NOTICED THE TRUCK WAS STILL RUNNING. AS HE BAC KED UP HE SAW SOMEONE SLUMPED OVER ON THE FRONT SEAT. HE CHECKED FOR PULSE & THERE WAS NONE. CPR WAS STARTED. IT IS BELIEVED HE STOPPED & TRIED TO PUT TRUCK IN PARK. FATAL HEART ATTACK.
EE BEGAN MISSING WORK FEB. 5, 2001
EE WAS WALKING ACROSS YARD AREA FROM SUB-STA TO PREHEATER TOWER BLDG. THERE WAS A CUST OF WIND WHICH CAUSED SOME DUST TO BECAME VALBORNE. HE WASHED OUT EYE AND THE EYE THE NEXT DAY 12-23-00 WA S GORE AND IRRITATED-WENT TO HOSPITAL EMERGERCY AND WAS GIVEN A PRESCRIPTION.
