Fall from machine

EE was changing the v-belts on the north log washer motor gearbox assembly. EE lost balance when wrench slipped off the motor mounting bolt ee was tightening and fell backwards through a 15 3/4 inch gap between the log washers. EE was conscious and talking and was told to remain in place until EMS arrived.

While cutting belt at tail pulley the blade slipped out and hit employees left arm. Left arm was positioned in a bad area when this happened. The cut was on EE's forearm 3 inches past base of thumb. Cut got fatty tissue and some mussel.

Employee was removing a conveyer belt when the right outrigger on the forklift they were using shifted downward causing the forks to hit an electrical box where they had their hand. The impact caused a fracture / laceration to their left thumb.

Employee stepped on a concrete floor scraper causing the wooden handle striking their thumb on 4/4/2025, medical treatment was sought later on 4/15/2025 and they were diagnosed with a strain to the thumb and was placed on restrictions and physical therapy.

Employee opened a toolbox drawer when it fell down onto their right thumb and tool box door causing a fracture to their right thumb

EE was cleaning under a conveyor belt when EE stood on the bucket of the skid steer EE was operating. EE began to cut part of the belt and material fell on EE causing EE to lose EE's balance and fell aprox. 24 inches on EE's left side injuring EE's shoulder, left hip and rib.

Electrician was removing spillage buildup on a decommissioned (de-energized) cable tray which had collapsed. While working to pry and break spillage buildup, the material released causing EE to hit the side of their hand on metal box adjacent to cable tray smashing the right small finger (pinky) between the box and the tool handle.

Employee was cutting the old rubber endcaps off of the spray bars at the Wash Plant. While cutting one of them, the retractable-blade utility knife they were using slipped off the rubber and contacted their left inner thigh. This caused a laceration about 1 inch long and 1 inch deep. 3 Sutures required.

The EE placed right hand inside a pin hole to remove grease before installing a hammer attachment on an excavator. The hammer attachment slipped pinching fingers.

EE was in the process of driving T post when right thumb struck the top of the post while holding the post driver handle. The EE was wearing cut level 4 gloves at the time of the incident.

EE was going up in the man-lift basket and approaching the railing of the screen 3 and 4 deck. EE had EE's left hand gripping the handrail of the man basket, EE's middle two fingers were curled over the rail of the basket. EE leaned forward and pinched EE's two fingers between handrail of basket and handrail of wash plant.

Employee and witness were rolling a section of conveyor belt off a roll that is suspended on a set of I-beams with a 3"" shaft. After cutting off the section needed they rolled the belt back up. The shaft fell off of the I-beam and struck employee on the shoulder, knocking them to the ground.

EE was removing barricade on a slight incline when EE slipped backwards. As EE was falling, EE placed hand behind to catch self. In doing this EE injured their finger on rocks in the area.

EE was performing maintenance at Wash Plant on C8 conveyor and wash screen 4 replacing a roller assembly. The assembly hit the handrail and caused it to rotate and pinching the EE's tip of ring finger between the roller assembly and tail pulley frame, causing a laceration to the finger.

The mounting plate for the Secondary Crusher (deflector plate) had built up material behind it creating a force on the plate. When the end of the plate was cut loose from the crusher housing, the plate rapidly released this tension like a spring and swung out impacting the mechanic. The mechanic suffered 2 lacerations on the face and as EE fell back landed on the tail bone.

EE had a tool in a portable vise on the back of a utility truck. Upon pressing with both hands on the lever to release the pressure of the vise it suddenly broke loose and EE put force on the inside of EE's right ring finger causing a fracture.

Employee called and stated they were working at cemex quarry pulling product (caps) from magazine and felt a sharp pain in back.

Employee was cleaning out a conveyor chute when EE reached down onto the flow of material to turn on a water valve and was struck on the right pinky finger by a 5 - 10"" rock.

EE works the evening shift and the injury occurred around 4am. The employee was shoveling material out from under a conveyor belt when ee hit a hard spot in the material and felt pain in elbow. The employee was taken to minor emergency. EE was released back to work with non-prescription medication and restrictions of motion to the right arm.

While lugging on a tire, tech was using crane hands to line up tire with bolt holes. Guiding tire into place ee applied down pressure on tire. The tire slipped in the loose gravel causing the boom to surge forward hitting the tech in back, causing ee to lose breath and pain in the lower back.

Employee was moving the chocks on a radial stacker and was bumped by another employee who was operating the skid steer. Employee thought EE was OK but EE went to the doctor the next day and had a hairline fracture to one rib.

Employee was changing out hammers on the crusher. He put one of the old hammers on the ground and left it standing up. it fell over and hit his foot behind his steel toe on top of his foot.

Fall from ladders

IW was standing on ladder installing a part on a 777 truck bed using truck hoist. IW thought brackets were engaged (not visible while mounting part) so released tension from hoist. Using a hammer to knock part down better bracket came loose and fell knocking the ladder out from IW. IW fell to ground catching self with right arm causing injury to right arm and shoulder

The employee was shoveling on the 10th, felt pain at end of shift, Employee worked until the 28th. At that time he came to the supervisor saying it had been hurting since the 10th and that he thought he could work through it. The employee was diagnosed with shoulder strain.

The employee stated that after climbing down the ladder of the water truck, when he went to turn to go to the back of the truck his leg planted when he twisted and he went to the ground hearing a pop in his knee. He refused to go to the doctor that day and on 12/1/2014 he asked for medical treatment and was diagnosed with a Sprain on his right knee with work restrictions.

EE was lifting at work and strained low back. He experience pain and was taken to Texas Medical Clinic. He was given prescriptions and light duty. EE returned a week later for follow-up and was feeling much better and able to touch toes.

Dizzy feeling, heart racing, feels weak, feels worse after eating, dehydration from working in extreme heat. Effects all over body. No equipment, nothing directly/indirectly effected this employee.

EE was working on the VSI to remove the table and had the crane attached to the table. The table was broken loose with a hyd jack and when pressure was taken off the jack, the table went down on the shaft. As the crane applied pressure, the table jerked off the shaft and rotated catching EE's foot against an anvil in the VSI.
