Employee was standing next to the hood doors on K2 Burner Floor when a 6"" metal braided hose that was being suspended broke loose causing it to swing striking the employee in the face.

Employee smashed EE's left index finger with a hammer
An operator was handing an impact wrench over to another operator that still had pneumatic pressure. When the receiving operator grab the socket end to take possession of the impact the operator handing it over accidently engaged the trigger.
EE felt a sharp pain near EE's sciatic nerve when EE pushed a plate.
We had a fire on a storage building that is used to charge electric golf carts for employees. We believe a charging station malfunctioned and got hot causing a fire on one of the golf carts that then spread to the surround carts and area. No injuries. Believed by the Fire Department who responded an MSHA Inspector to be an electrical fire.
EE visited EE's personal doctor due to feel strength loss in EE's grip on both hands. EE was told EE had carpal tunnel in both hands caused by repetitive motion. EE has worked for CEMEX USA as a Production Assistant Laborer for the last 16 years.

Injured Employee burnt EE's hands while putting a cover in an open pipe in K1 Preheat Tower.
Employee stepped across a drainage area, slipped on some mud and fell backwards injuring their right elbow
The employee stepped off a walkway approximately one foot in height. EE stepped into some mud and material with their right foot that had collected in a drainage area. EE's foot slipped out from under EE, straining their left knee.
A layer of concrete dust and debris had collected on silo roof obscuring hatches lids, and other obstructions. Walking on roof tripped on one of the obstructions. Fell to right knee and struck something in the dust and twisted some on the way down. Local clinic did x-rays but nothing broken. Received brace for sprain to unspecified site of right knee.
Injured employee used a bar hook with a T handle to snare the cable of an air hoist to pull it towards themself to retrieve the ball. The employee's left index finger was caught in a pinch point between the bar hook T handle and the rosette of a vertical scaffold leg. This resulted in a pinch laceration requiring stitches
Contractor miner was tasked with putting the guards that go over a feed belt back on. After leaning over to tighten the guard EE relaxed their weight on the handrail and the horizontal arm on the handrail gave. The contractor then lost their balance and their torso fell on toe plate. The toe plate was 45 degrees outward when the accident occurred
EE was working inside of the K1 Mill Welding out bolts, while working in the lower section of the table a hammer fell through a 3x2 foot opening from approximately 8ft and struck EE's rear right shoulder. When the accident occurred plywood was covering the opening but the hammer slipped through a small section of the opening.
A fire broke out on the K1 burner floor coal system when a spark ignited a wooden shed that had accumulated coal.
Tires in the tire station of K1 Preheat Tower caught on fire.
Maintenance Mechanic was replacing an elevator bucket, when handling the bucket, the bucket shifted causing a laceration on EE's left ring finger.
Production Assistant employee was called to reset a disconnect in Electrical Room 5 when the first attempt failed to reset the panel. The employee then proceeded to pull the handle, halfway through the motion the panel blew up causing an arc flash striking and electrocuting the employee on EE's left hand.

Maintenance Mechanic was walking towards the lubrication shack where the EE rolled an ankle due to stepping on slippery conditions, EE was then taken to the clinic for further evaluation. EE was then diagnosed with a hairline fracture in right foot.
Employee was removing rotor from Clay Crusher, rotor shifted and caused right thumb laceration and fracture.
Troubleshooting Silo 12 gate and hit EE's head on the lower part of the guard, causing a laceration.
Injured was removing a section of duct with mobile crane. When duct was free the duct struck the injured in the right arm and jaw.
A locking nut on the screw on coal mill 1 backed out allowing the shaft to float and to rub on the housing. This allowed coke to pass through the gap which was now being heated by friction. This caused the coke to smolder.

While walking down the stairs at the K1 Homo Silo, EE stated that EE felt pain on the outside of left knee. On 09/27/2021 after self care failed, EE was seen at a clinic where EE was given restrictions and a knee brace.
EE picked up a chipping gun from a rack and felt pain in lower back. On 08/02/2021 EE was seen by a Dr. and given restrictions for a strained lower back.
While removing the bolts for the uphill thrust roller on K1, EE had their fingers pinched between an impact drill and the structure of the thrust roller resulting in 6 stitches to the 4th and 5th fingers on left hand.
While walking down stairs on K2 PHT EE slipped on ice causing shoulder bruise requiring self care. On 3/8/2021 injury re-evaluated and put on restricted duty.
EE was cleaning K2 clinker pit and stood on top of clinker. Heat transferred through their boot and caused a burn on the bottom of their left foot.
EE stepped off the bottom stair on a staircase awkwardly and sprained their right ankle.

EE was walking down stairs in the coal unloading area. Their bodyweight caused their arm to lock, and the EE felt pain in their shoulder. EE was referred for Physical Therapy on 10/26/20 after the pain had not fully subsided.
While tightening a union on a pipe, the wrench came loose causing the EE to fall backwards causing a small fracture to their Fibula on their right leg.
While working on the K2 PHT, the employee failed to close their PAPR shield all the way. Material and hot water was discharged from an 8 inch inspection door causing burns to the face and eyes
While climbing a ladder on a scaffold, the employee lost their grip and fell three rungs onto the ground causing a broken left foot.
EE stated that when getting out of buggy at the end of the shift they felt pain in left leg. The employee did not report this incident until 4/12/2020. On 4/13/2020 EE was diagnosed with a knee sprain.
An employee was closing an inspection door on the K1 pre heater tower. While closing the door the employee's right middle finger was pinched between the latch of the door and the handle. The employee had 2 pairs of gloves on, but still sustained a laceration to EE's finger which required sutures to close the wound.

Employee was poking a hole in the K1 tower using a bar and a 12.5 lb. sledgehammer. The employee who was injured was holding the bar, while EE's partner swung the hammer. The hammer missed the bar and struck the employee in the left ring finger. The injury resulted in the removal of the tip of the left ring finger.
Miner was walking between work areas when a piece of dust got into EE's eye behind the safety glasses. The miner flushed the eye and removed the dust. About an hour later there was still discomfort. Miner was sent to the clinic where a 2 mm abrasion was found on the eye.
Employee had hot material from cement tower blown into face. Minor burns on face. Some material had to be removed from EE's left eye.
2019 employee was injured while patching holes on the singulator of the TDF. During The welding process EE was in the sitting position with both legs extended in front of them. After 4 minutes they went to stand up and could not bend the right leg. When they finally did they heard three pops. EE came out of the TDF and rested a while, but was experiencing pain in the right knee.
Employee was attempting to close the door on rotor feeder of the Raw Mill #1. The employee started to lift up on the door, when the door stuck and the employee felt a pain in left shoulder. The employee was prescribed physical therapy on 7/16.
Production mechanic was changing out the sleds under the impact zone of conveyor. Mechanic flipped the sled frame over with assistance from contract mechanic to finish installation. During this process mechanic felt a pull and then noticed a knot around EE's groin area.
Employee was moving I-beams when EE felt a pop in EE's right shoulder.
There was a notification of confirmed hearing shift for an electrician who refuses to fill out the questionnaire identifying if this was a work related shift. Previous testing shows this position exposure to be under 85Dba.
Electrician was changing out motor for a magnet belt during the day. EE went home and felt swelling in right knee. EE found a small gauge 1/2 inch wire in the knee. After going to the med clinic EE was found to have septic arthritis of the right knee.
Mechanic was removing bags and cages from the K1 Main bag house in a standing to kneeling position. EE developed Achilles Tendinopathy from this repetitive motion.
Employee was removing the hood of their 40 cal suit used to rack out 4160 switch gear when they felt a foreign particle drop in their eye. After examination by local clinic they were found to have a 2mm X 1mm abrasion above the right cornea. No particle was found, and no time was lost from work.
Employee was trapped in K-2 elevator between the 7th and 8th floor due to a mechanical malfunction. Employee stayed in communication through cell phone and radio. There were no health concerns and employee stayed in good spirits.

During the lighting of the K-1 Burner we had a buildup of gas causing an unplanned Ignition of that gas.
Contractor overheated and got too hot. EE was sweating profusely and shaking. EE had taken Tylenol earlier.
Contractor was welding toe plate on a cat walk for the belt conveyor to the raw mill. Contractor told co-worker that contractor was having trouble breathing. They went down stairs to get a drink & see if contractor could catch breath. Down stairs contractor said arm was numb so CEMEX called paramedics & contractor was taken to the hospital. Contractor was released the next day.
The CO2 system discharged on the dust collector. We called the fire department to assist. No open flame was seen but the internal temperatures caused the internal bags to start smoldering. There were no exposure to employees.
Employee reported slipping and falling on 01-06-2018. EE refused medical treatment and said was fine. On 3-1-2018 employee stated that the shoulder has not gotten better. We took EE to the doctor where they said EE had a contusion. EE was put on restricted duty and prescribed PT.

Employee originally felt a pop in shoulder on 6-29-17. EE did not receive any restrictions from the doctor. In a week EE was still hurting went back to the doctor on 07-06-2017 and received PT for 3 weeks. Employee was in a man basket and reached out to a light fixture and felt a pop in the shoulder.
Employee was using a 4"" grinder with a cutting wheel to cut angle iron when grinder bound up causing it to kick back creating a laceration on the employees mid left thigh

Employee was arriving to work on motorcycle. EE was making a right hand turn very slowly to park. the back wheel of the bike slid and the employee laid the bike down pinning EE's right ankle under the saddle bag. Employee suffered a fracture to ankle.

Amilcar was working in open pipe under kiln. Cement got stuck in pipe. He used sledgehammer to hit pipe to loosen cement. When cement loosened it came out with pressure and hot water behind it. The hot water saturated his jeans. Blisters formed on both legs in spots from thighs to ankles.
The employee was striking an airslide to get material moving through it. There was a pipe nipple next to him on the airslide that was for clean out. The nipple cap popped off and struck the employee in the face shield knocking off his hard hat and safety glasses. the employee got cement dust in both eye's.
Employee was walking on even ground through the plant, he did not trip, he did not stumble, he did not roll his ankle. He heard a pop in his foot under his ankle. Employee has sprained foot.
EE was moving a ladder while looking up & stepping backwards. He stepped into the center of the raw mill table, falling & landing on his right side at the torso area. He came to the office at 9:09 am & said he had fallen & wanted to go to the hospital. He was having trouble taking deep breaths.

Employee was cleaning the pre heater tower through a 2"" opening when a lower level air blaster discharged. The material shot out of the port. the employee was not wearing the required PPE and received burns to his left wrist and hand.
Employee was using a rivet buster to break up material behind the maintenece shop for aproximatly 4 hours. Next day the employee complained of burning sinsation in his shoulders. The employee went to the med clinic where he was diagnosed with a right shoulder strain. The employee returned to work with work restrictions of no overhead lifting and PT 3 times a week for 2 weeks.
Employee working at Cemex Balcones in Lyston Building moving rocks from Quarry to plant using skid steer. Twisted right ankle stepping down from skid steer. Taken to Texas Med Clinic, diagnosed with sprained ankle, given boot to wear with crutches as needed and placed on restricted duty. Cemex Balcones will not allow employee back to work until able to wear steel toe boots.

The employee was using a pry bar to raise a water line while another employee was uncoupling it. When the line came apart the weight was more than the employee was aware of and the pressure caused the bar to come up causing a back strain.

Employee was walking through the plant when a gust of wind put a spec of dust in his eye. Employee was taken to med clinic and recieved prescription eye drops.
Working on the DB elevator on 09/27/12. He went to his doctor for a physical on 10/01/12. He stated to doctor shoulder had been hurt and they x-rayed and MRI, employee did not say anything to me until he got back MRI and x-ray results on 10/05/12. He was scheduled off the weekend, on Monday the 8th we took to doctor where he was given restrictions. Date reportable was 10/08/12.
employee was using a 6' metal bar to break up a build up of material in a chute. the material broke apart and the employee could not stop his momentum and his hand hit the seal for the chute door. The employee received five stitches to his right ring finger and 8 stitches to his middle finger. Both fingers were also fractured.
EE was working in the trailer tipper area. He was loading tires onto a belt. He bent down to pull a tire that was stuck. As he pulled the tire to loosed it, he twisted his back and felt pain immediately. EE continued to work. He called his general foreman to inform him and he was instructed to go to dr. due to pain. He was given prescriptions. He is on restriction.
EE was retrieving a sample of raw meal material in a metal 1 gal bucket. On the way back to the lab the ee slipped in mud and when correcting himself he brought his hand up and the material in the bucket contacted his face. The material was hot, the ee received 1st & 2nd deg burns to face, and received minor burn to right eye. EE received prescription eye drops and cream for face
While cleaning about a plugged vessel with an air lance, the vessel flushed dust out and contacted the employee in the face getting dust in his left eye. The employee washed out his eye and completed his shift. Then he went to the clinic and a floating particle was removed. He received prescription eye drops.

EE was walking in the plant area KT Pit #2. As he was returning from water break, EE was walking down a step when he twisted his right ankle. His supervisor transported him to a local emergency room. EE was diagnosed with no broken bones but has a twisted ankle and the dr. told him to take Advil for pain.
Employee and another employee were relocating metal cabinets. When placing one of the cabinets down employee cut his hand on the frame of the cabinet
EE squatted down to lift 3 cardox tubes. He placed them on his shoulder. When EE stood up he felt a pain in groin area.
EE was climbing ladder to enter cab of loader. EE was reaching for door while holding on to ladder with his gloved left hand. The glove had grease on it causing him to lose his grip and to fall backwards. EE's left foot hit ground first which was followed by his landing on his back. EE used his left hand to break his fall.

Piece of steel fell on the miners left foot just above the metataral area.
While exiting K-1 clinker breaker area, hot material went down the miner's back and both pants legs. Experienced minor burns to his back and both legs. Work restrictions began upon his return to scheduled work on 9/20/2010.
While removing buildup from the Burner pipe the miner stepped back, lost his balance and fell from a platform on a three foot tall rolling stair. Safety chain was not in place. Miner didn't report injury until the next day and didn't receive work restrictions until 09/06/10.
While driving in bar gates with a sledge hammer miner's finger was caught between the hammer handle and a steel beam.
Removing scraper from Raw Mill table miner lifted and twisted while moving the scraper, and felt a strain in his groin area.

Slipped on cement slurry while walking to Finish Mill, fell on left hand spraining thumb. Medical treatment not requested until 10/14/09, splinted thumb/wrist.
Miner was investigating why a dust collector's screw conveyor stopped. He turned off conveyor's power but didn't follow procedure to turn off/lockout the co-located rotary feeder's power. He then put his hand into the inspection port to see if there was any material buildup. The still moving rotary feeder struck the index & middle finger of his right hand, & lacerated them.

While performing a walk through inspection of the pre-heater tower, non-metalic dust was blown into the right eye, by the wind. Safety glasses with side shields were being worn. The foriegn body was embedded in eye by rubbing. It was removed by a local physician, who issued a prescripition eye drop. There is no concern by the physician for permenant damage.
Employee was grinding inside the Clinker Silo and when he finished, he raised his faceshield and a large amount of debris went into his eyes and on his face.
Safety pin on Finish Mill ball dump chute broke causing the handle to fall striking miner's left hand, lacerating three fingers.
The injured man notified the other worker that he was getting down from the mixer, but returned just as the other employee closed the lid/hatch to the shotcrete mixer. The injured man got his ring finger tip smashed when the lid closed.
Employee was carrying a rivet buster into kiln when he slipped and caught himself with his hands. Rivet buster fell onto his left ring finger and crushed it.
Employee was removing boards on a scaffold. He was taken to the Doctor, his eye was flushed and was perscribed eye drops.

While carrying a bag of trash, he passed by an air horn, lost his balance and fell on his buttock causing unknown injury.
No conditions contributed to the incident. Miner experienced respiratory difficulties.
Carying rebar cage to hole, cage rolled while carrying and cut his arm.
Mechanic was installing hydraulic cylinder onto excavator. He removed pin to re-align. The other end of cylinder was suspended by rigging; the other end of cylinder slipped out of the pin socket, swung away suspended then swung back and struck mechanic on the jaw causing fracture to jaw. The mechanic stepped down from ladder and asked for assistance.
Employee was using hand-power tool, drilling into wood; drilling bit unexpectedly became stuck into wood, then ""freed-up"", causing a twisting motion to hand/wrist area causing small bone fracture located on topside of right hand, near right ring finger.
Employee was positioning forks of forklift. He had his hand in the pinch point area when the other employee lt go of the fork resulting in a fracture to (R) ring finger.

Miner missed a step while climbing off a loader and stepped onto uneven ground twisting his left knee. No restricted duty; however, his leg was placed in a temporary immobilizer. Miner did not request first aid or medical treatment until 09/23/2005.
While lowering a 10' X 24"" Finish Mill airslide the rope used to support and lower the airslide broke. The airslide struck the miner's lower right leg.
Employee was using a ""T"" handle pipe to poke the Preheat Tower. Because of the improper placement of EE hands on the tool, when the material gave way, EE left hand struck to extended port structure.

Co-worker was attempting to align a metal liner in a Finish Mill, and while striking the casting with a metal sledgehammer, a small piece of the metal casting struck the injured miner and became embedded in the miners neck. Due to the position of the metal minor surgery was required to remove the embedded metal.
MINER INJURED HIS LEG WHEN A TEMPORARILY WELDED SUPPORT ANGLE IRON GAVE WAY AND THE TEMPORARY PLATFORM SHIFTED SLIGHTLY. WHEN THE MINER JUMPED OFF THE SUPPORT, HE LACERATED HIS LEG ON THE COR NER OF THE ANGLE IRON.

WHILE HOLDING LARGE PIECE OF METAL, BEING MOVED WITH A HOIST CHAIN MOVED PINNING RIGHT HAND BETWEEN CHAIN AND METAL CAUSING INDEX AND MIDDLE FINGER TO BE LACERATED. GLOVES WERE WORN; HOWEVER ANGLE OF CHAIN AND METAL CREATED UNDUE PRESSURE ON THE CHAIN WHEN IT WAS ACTIVATED.
WHILE LIFTING A GUIDE RAIL, WITH 4 OTHERS, HE LOST HIS FOOTING. INJURY WAS REPORTED ON 5/22/03; HOWEVER, HE DID NOT SEEK MEDICAL ATTENTION UNTIL 6/13/03.

UNKNOWN. FELT PAIN AND ITCHING ON LT. FOREARM. RAISED SHIRT TO DISCOVER RED SPOT. SUSPECTED INSECT BITE, NOT FROM A SPIDER (BROWN RECLUSE OR BLACK WIDOW).
WHILE USING A POLE TO REMOVE MATERIAL FROM THE INTERIOR WALLS OF THE PRE-HEAT TOWER A LARGE PIECE OF THE MATERIAL FELL FROM ABOVE STRIKING THE END OF THE METAL POLE PLACED THROUGH THE PORTAL. THIS FORCED THE METAL POLE UPWARD CAUSING HIS FINGER TO BE CAUGHT BETWEEN THE POLE AND A STEEL DOOR.
EYE BECAME IRRITATED AFTER EE LEFT PLANT & ARRIVED HOME. HE TRIED TO WASH MATERIAL FROM EYE WITHOUT SUCCESS. THE FOLLOWING DAY HE CONTINUED TO HAVE IRRIATION & WAS SENT FOR MEDICAL ASSISTANCE IN REMOVING THE MATERIAL.
EE WHILE OPERATING VACUUM TRUCK WAS STRUCK ON RIGHT EAR, RESULTING IN A SMALL LACERATION, WHEN THE 8"" VACUUM HOSE CAME LOOSE FROM THE PIPE TO WHICH IT HAD BEEN ATTACHED.
EE STOOD UP TOO SOON WHEN EXITING FROM UNDER TEMPORARY SCAFFOLDING ERECTED FOR A MAINTENANCE PROJECT & HIS HARD HAD STRUCK THE OVERHEAD PLATFORM, SPRAINING HIS NECK. HE RECEIVED MEDICAL TREATM ENT ONE WEEK AFTER THE INCIDENT.
