Employee was going through a brattice man door at cross cut 621. She opened the door, started through the door and the air caught the door. She was trying to hold the door and it caused her to strain her right shoulder and lower left leg trying to protect herself. We could no longer accommodate work restrictions starting on 2/19/2015.

NOT AN ACCIDENT: Employee arrived on #1 LW dinner hole. The foreman turned around and saw the employee fall to the ground and called to him and tried to open his airway. CPR and AED were used but there was no response. They got the employee outside and the helicopter paramedics tried to revive him and failed. He was pronounced dead at 1:12 A.M. (Possible Heart Attack)

NON- ACCIDENT-- A Continuous Miner Supervisor was talking with another employee and fell to the ground with a possible heart attack. CPR was given as he was transported to the surface. He was pronounced dead by the Helicopter and Ambulance personnel at approximately 1:15 A.M.

Employee was reaching into his tool bucket when he was bit by something. He thought it was a spider and shook it off. At lunch he decided he needed to go to the hospital. The hospital later decided he had been bit by a snake.

This was not an accident. An employee came into the Preperation Plant Office and sat down in a chair and bent over. Another employee laid him onto the office floor. The employee passed away while being transported by helicopter to the hospital. It appears that he died from natural causes. We are still waiting for an official work from the Medical Examiner.
