Employee was going through a brattice man door at cross cut 621. She opened the door, started through the door and the air caught the door. She was trying to hold the door and it caused her to strain her right shoulder and lower left leg trying to protect herself. We could no longer accommodate work restrictions starting on 2/19/2015.
Employee was lying on his back, micing bores on 12CM12 mainframe CM97. A gust of wind blew debris from yard into his right side of face and into his eye. Received prescription.
Employee stated that he pulled his shoulder out of socket. Treated by doctor. Employee has a rotator cuff tear and has a partial torn bicep tendon. His first date of loss is 4/29/2013.
Employee was walking to Man bus, when he felt a pop and pain in ankle.
The employee was riding in man bus to his work location, once arriving at his work location he said his chest was hurting. At that time we evaluated him and brought him back to the bottom. Later to find out it was pancreatic.
The employee had chest pains and was transported to UAB Hospital.
Employee stated that he walked out of slope door walking out-by and air forced coal dust or dirt got into his right eye. He was sent out for treatment and received antibiotic ointment to go into his right eye.
NOT AN ACCIDENT: Employee arrived on #1 LW dinner hole. The foreman turned around and saw the employee fall to the ground and called to him and tried to open his airway. CPR and AED were used but there was no response. They got the employee outside and the helicopter paramedics tried to revive him and failed. He was pronounced dead at 1:12 A.M. (Possible Heart Attack)
Employee was walking to his toolbox and felt his knee pop. EMPLOYEE DID NOT START LOSING TIME UNTIL 4/7/2010 DUE TO INCREASED PAIN.
Employee was traveling #2 Longwall Bleeders and suffered heat related injury.
NON- ACCIDENT-- A Continuous Miner Supervisor was talking with another employee and fell to the ground with a possible heart attack. CPR was given as he was transported to the surface. He was pronounced dead by the Helicopter and Ambulance personnel at approximately 1:15 A.M.
Employee was cleaning dust build up on welder sled and dust particles blew into his eye. He was wearing his safety glasses.
Employee was reaching into his tool bucket when he was bit by something. He thought it was a spider and shook it off. At lunch he decided he needed to go to the hospital. The hospital later decided he had been bit by a snake.
This was not an accident. An employee came into the Preperation Plant Office and sat down in a chair and bent over. Another employee laid him onto the office floor. The employee passed away while being transported by helicopter to the hospital. It appears that he died from natural causes. We are still waiting for an official work from the Medical Examiner.
Walking up stairs. Back started hurting.
While cutting out at the tailgate, employee crawled over the gob on #144 Shield and when he got up he had pain in low back.
Employee was assisting coworker in drilling with sinking hammer. Employee collapsed. Possible heart attack.
Employee had a long time history of heart related issues. NO SPECIFIC EVENT. Transported to hospital, died in route.
Before EE's shift began, he was in the bathroom and died from natural causes per investigation by the Alabama Department of Forensic Sciences.
ALLEDGED: EMPLOYEE STATED THAT HE WAS PUTTING ON GLOVES AND FELT STINGING IN RIGHT HAND. REPORTABLE ONLY DUE TO EXTREME SWELLING OF RIGHT HAND TO BE LANCED. EMPLOYEE DID NOT GO TO THE DOCTOR UNTIL 8/10/04 AND DID NOT MISS ANY WORK.
EMPLOYEE WAS BENDING LOW AT THE KNEES TO GO THROUGH BACK DROP & FELT PAIN IN THE RIGHT KNEE. *EMPLOYEE DID NOT START LOSING TIME UNTIL 7/10/04 DUE TO SURGERY*
EMPLOYEE LOST TIME AGAIN ON 10/12/01 AND 10/13/01 DUE TO INJECTIONS IN THE KNEE.
EMPLOYEE STARTED LOSING TIME AGAIN ON 10/10/01 DUE TO INCREASED PAIN.
EMPLOYEE AWAY FROM WORK BEGINNING 9/28/01
EMPLOYEE HAD A HEART ATTACK WHILE UNDERGROUND. ALL EFFORTS OF RESUSCITATION FAILED. HE WAS PRONOUNCED DEAD BY A DOCTOR WHO WAS AT THE MINE SITE.
PIECE OF METAL IN EYE, EMPLOYEE WAS WIPING SWEAT FROM AROUND EYES AND GOT METAL IN HIS EYE.
