Employee strained low back lifting an I-beam. **Employee did not lose time until 3/19/07 due to injection in back.**

Employee was working on the pan line of the longwall when a rock hung up on the pan line and flew up and hit the employee above his upper lip. (Laceration required a suture above the upper lip.)

A roof fall occurred at approximately 11:10 pm on Tuesday, December 5, 2006 outby the longwall face. The roof fall fell to the main roof and is approximately 20 feet wide by 45 feet long. The fall is located between the longwall face and spad 10082 in the No.6 entry. The area was supported with 6-foot resin bolts. (MSHA field supervisor was notified at 11:22 pm on 12/5/06.)

A roof fall occurred at 9:32 pm on Tuesday October 24, 2006 in the 2-east main belt entry. The roof fall is located between spads 10054 and 10068 and is approx 20' wide X 6' high and 70' long. This area was supported with 6' resin bolts at the time of the roof fall.

Employee was getting off of a scoop when employee twisted his right ankle on a rock. After an MRI was done on 8-21-06 it showed the he had a small bone fracture to right ankle. (Did not start missing work until 8/22/06)(EE started losing time again on 12/21/06 due to continued pain by misc mail #6 12/22/06)

Employee was attempting to open a set of big air lock doors all the way when the chain came loose and his left arm was caught between the air lock doors (it was not found out that the employee had a small fracture to his left lower arm until his doctor appointment on 8-15-06).

Damage to the north production shaft skip bat wing assembly unit was discovered by an outside employee at approximately 9:30 pm. The skip was removed from service until repairs could be made. The production shaft was down until Tuesday eve shift on 8/1/06.

Employee was going through mandoor when employee put his hand on a 2"" pipe to brace himself. When ee let go of the pipe the pipe flew up and hit the ee above the right eye. (Laceration above right eye that required 25 sutures).

EE was walking down the longwall face and stepped in a hole and twisted his left knee.

Roof fall was discovered on the tailgate side on N-panel Longwall set-up at approx. 11:53 pm between spads 14690 & 14691. After investigation it was determined that the roof fall was above the roof bolt anchorage zone. An MSHA field office supervisor was notified at 12:33 am. No personnel were affected, no equipment or ventilation controls were damaged.

Employee strained abdomen area as he was pulling himself out of scoop. (He did not start missing work until 7/10/06 due to hernia surgery.)

DID NOT START LOSING TIME UNTIL 01/12/07. While inspecting Prep Plan, slipped on floc and fell on left knee. Did not start losing time until 1/12/07 when he had surgery.

Employee was moving hose from the rock binder glue tube when some rock binder glue splashed into employee's left eye. Employee was wearing safety glasses at the time of the accident.

EE was drilling through a pot when pot broke and fell hitting EE and causing a laceration to lower arm that required 5 sutures.

A ingnition occured on the No 6 section at approx 5:05 am on monday morning 3/20/06. The miner #81 had cut approx 13 feet and had started cleaning up in the # 2 entry when the ingnition occured on the right rib. A methane test was made before mining started in the face 0.4%to0.6%. The lasted 4 to 5 minutes before extinguished.

EE was lifting forks to put back on to Lo-Trac when he felt pain in Right shoulder. EE did not start missing work due to this accident until 3/22/06.

AT 705, A METHANE TEST WAS MADE & 0.0 WAS ALL THAT THE SPOTTER READ. AT THAT TIME THE PREP PLANT MECHANIC STARTED WELDING. THAT IS WHEN HE FELT HEAT ON HIS HAND. HE STOPPED, LOOKED AROUND & STARTED AGAIN. IGNITION OCCURRED.

At the bottom two employees were taking an old bottom gate down and when it came loose the gate was too much for the employees to hold onto. At the same time one of the employee's right foot was under it and mashed his right foot.

Employee and another employee were unloading electrical part from a pick up truck and the other employee picked up on his end and pinched injured'd index finger under the electrical part they were unloading.

On Friday, 5/27/05, water inflow into No. 5 Mine exceeded pumping capacity. No employees other than those involved in water pumping were allowed in the mine after the Friday Owl Shift. Pumping efforts continued until 8:15 AM on Monday, 5/30/05. All power into the mine and the 5-3 Fan were then shut off. The mine is currently idle.

(did not start missing work unitl 4-10-06) He last visit to a Medical facility on his shoulder and neck was on 1-5-06. He had a heart attack an had to undergo by pass surgery and on 4-10-06 had been released to undergo surgery to his neck.

EMPLOYEE HAD ONE FOOT ON THE RAM BAR ON 146 SHIELD. THE SHIELDS STARTED TO PUSH THE TAIL GATE DRIVE. THAT IS WHEN HE DISCOVERED HE HAD HIS FOOT UNDER THE PAN LINE AND DRAW BAR. THAT IS WHAT CAUSED THE INJURY TO HIS LEFT TOES.
