Showing 50 of 58A rock became lodged between the blade arm of the dozer and frame. When removing the stone it slid between the blade and frame causing it to roll backwards resulting in a pinched finger. The resulting injury was an impact laceration with light duty for 3 work days.
While trying to change blade in utility knife, EE was using EE's pocket knife to undo the screw that holds the knife together. While trying to unscrew it, the catch did not latch on EE's pocket knife causing the blade to close on EE's right index finger.
EE was driving in a wooden wedge, the wedge was at an awkward angle. When hammering with the right hand, EE missed and hit pinky finger on left hand. EE got 10 stitches on pinky finger.
EE was attempting to install screens in the wet deck. Using a pry bar EE was under the wet deck prying the overhead screen up when the crowbar slipped. EE grabbed the overhead beam to keep self from falling foreword. Left shoulder injury to be determined
While cutting conveyor belt with razor knife, employee slipped and cut wrist.
While removing bearing, employee had a sliver of steel pierce glove cutting hand.
Fell off second rung of 4 foot ladder while reaching over head.
Steel chip entered eye while grinding.
Employee was clearing out water pipes, he was pulling a stick out of the drain, the stick snapped back at him and caught him in the upper lip.
While moving a 55 gallon drum of oil onto the forks of a Bobcat the forks moved and tipped over the drum and pinned the employee's finger between the drum and another 55 gallon drum nearby.
The employee was helping to change screens on the crushing plant. The screen slipped and crushed his left finger between the screen and the spring on the vibrator.
vibration weakened the side grab rails of the ladder at the weld attachment points.
Employee was using his right hand to cut rubber witha utility knife, the knife slipped from the rubber and went over his left hand causing a cut on his knuckles. Stitches were required and employee returned to work later.
The employee was moving an idler roller on the crushing plant and it fell and smashed the employee's finger between the idler roller and the man-lift.
The employee was dismantling a 5' crusher. He was standing on a crossbeam and lost his balance and fell approximately 10' to a concrete pad below. He hit his head and cracked his hard hat.
Employee was attempting to break a corroded bolt free and his elbow hit a piece of steel.
Employee was reaching over railing to grab sticks off screen deck and disupted a previous injury to ribs (previous injury w/ different employee).
Allen Copley was welding and grinding on the frame of a loader, he had his safety glasses on, but got a metal sliver in his left eye.
Employee was bent over and twisting, moving the cribbing(8X8 block) when he felt a pull in his stomach.
Employee, who was using the water truck to water the road in the pit area, was washing spillage from underneath the 7'Crusher pedestal when he needed more hose to complete the task. He yanked on the hose causing his feet to slip in the mud, falling onto the frame of the crusher pedestal.
HEARING LOSS
The jaw crusher had a rock bridge the opening to the crusher cavity. The employee shut down the crusher and began to attempt to clear the crusher. The employee got his left had pinned between a rock and the grizzle while trying to lift the rock out of the crusher, jamming his ring finger
EE was straightening up screens & organizing warehousing area. He injured his lower back area by picking up a screen. he was not in correct position and he twisted while pulling and lifting screen. the screen was 4' x 8' and weighty about 125 lbs
EE WAS LOADING A BROKEN SHEAVE INTO A LOADER BUCKET, SUPERVISOR, CAME OVER T HELP PUSH THE SHEAVE INTO THE BUCKET ON ONE SIDE AND IT FELL ONTO HIS HAND.
EE WAS WORKING WITH ANOTHER ON THE CRANE. THEY WERE REPAIRING THE SHIVE ON THE CRANE BOOM. THEY WERE PUTTING THE PIN BACK IN. INJURED HAD HIS HAND BETWEEN THE PIN AND THE CRANE AND WHEN OTHER HIT THE PIN IT BROKE FREE AND HIT INJURED'S RIGHT HAND CUTTING THE RIGHT MIDDLE AND INDEX FINGERS.
SHOVELING SAND UNDER TAIL PULLEY. HAND STRUCK SHARP PIECE OF STEEL ON TAIL PULLEY GUARD.
EMPLOYEE WAS OPENING A OIL BARREL AND HIS HAND SLIPPED AND CUT HIS FINGER.
EE WAS REMOVING CONCAVES FROM THE PRIMARY CRUSHER, HE WAS STRIKING THE CORNER OF A CONCAVE WITH A SLEDGEHAMMER. THE CORNER FRAGMENTED AND A PIECE OF STEEL STRUCK & BECAME LODGE IN HIS CHEEK.
EE WAS CHANGING THE SCREENS ON OCT 28, HE LIFTED A SCREEN INTO PLACE WITH A SCREEN HOOK. THE SCREEN BEVAME WEDGED AND HE TRIED TO FREE IT BY HAND. THE SCREEN SLID AND CAUGHT HIS HAND BETWEEN THE SCREEN AND THE SIDE RAIL. ON NOV 6TH HE WAS IN THE PROCESS OF TIGHTENING DOWN A SCREEN WITH AN OPEN ENDED WRENCH, IT SLIPPED AND SMASHED HISHAND AGAIN. HE REPORTED IT TO OFFICE.
EE CAME TO MANAGEMENT ON NOV 6TH TO REPORT THAT EITHER ON OCT 19TH OR 26TH WHILE WORKING ON THE PRIMARY CRUSHER HE WAS IN THE PROCESS OF RELINING A STONE CHUTE. HE HAD REMOVED THE OLD CHUTE & WAS PULLING THE NEW LINER UP WITH A CHAIN. HE FELT A PAIN IN HIS BACK. SINCE THAT TIME HE HAS BEEN EXPERIENCING NUMBNESS IN BOTH HIS HANDS.
EMPLOYEE HAD BEEN AT THE SECONDARY TOWER OF THE PRIMARY CRUSHER TALKING TO THE OPERATOR. HE LEFT AND WENT OUT OF THE BOOTH AND FELL DOWN THE STAIRS LEADING TO THE BASE OF THE TOWER.
EE WAS TRYING TO REMOVE A ROCK FROM A PULLEY ON THE CONVEYOR. HE WAS STRIKING A BAR WITH A HAMMER AND MISSED THE BAR AND HIT HIS RIGHT WRIST.
EE WAS SHOVELING STONE FROM SINGLE SCREW CRUSHER, HE HIT HIS PINKY FINGER ON A PIECE OF METAL ON THE SCREW AND TORE OFF PART OF HIS FINGERNAIL.
LIFTING ROCK WHILE IN AWKWARD POSITION.
EMPLOYEE TRIED TO PULL PIECE OF RUBBER FROM THE GROUND NOT REALIZING IT WAS FROZEN IN GROUND.
EE WAS RETURNING TO HIS TRUCK AFTER GETTING A SALES TICKET FROM THE SCALEPERSON HE WAS WALKING ACROSS THE SCALE AND HE TRIPPED OVER THE LIFTING EYE OF A MANHOLE COVER ONT HE BOW SCALE. HE TRIP PED AND FELL INTO A WOOD RAILING ATTACHED TO THECATWALK OF THE SCALE. HE BRUISED HIS RIGHT ARM.
EE BENT OVER TO LIFT AN OXYGEN TANK AND LIFTED THE WRONG WAS AND HIS BACK BEGAN TO HURT.
EE WAS WORKING IN THE WELDING SLOP AND WAS GRINDING THE SLAG OF STEEL. WHEN HE SET THE GRINDER DOWN ON THE GROUND, THE WHEEL WAS STILL SPINNING AND IT SLIPPED AND SCRAPED ACROSS HIS LEG, CAUSI NG A GASH IN HIS LEG.
EE FUELING LOADER. FELL COMING DOWN LADDER.
EMPLOYEE WAS CLIMBING DOWN FROM THE SCREEN AREA OF THE PLANT. HE GOT HIS FOOT TANGLED AND FELL DOWN THE STAIRS AND LANDED ON HIS REAR END ON THE CATWALK.
EE WAS UNLOADING OIL DRUMS FROM HIS TRUCK, HE LOST HIS FOOTING AND THE OIL DRUM FELL ON HIS FOOT.
WHILE INSPECTING THE CRUSHER FOR A PROBLEM, EE STEPPED ON A STONE AND STUMBLED AND TWISTED HIS RIGHT KNEE.
SAND WENT IN EYE WHILE GREASING LOADER.
INSTALLING TAIL PULLEY FINGER CAUGHT BETWEEN BEARING AND COLLAR.
EE WAS WORKING IN THE GARAGE AREA CHANGING STEEL IN THE TOP OF A CRUSHER. HE CLIMBED ON TOP OF THE TOP OF THE CRUSHER WITH A BAR TO ADJUST THE POSITION OF THE LINER. WHILE TRYING TO PRY THE LI NER, THE BAR SLIPPED AND HE LOST HIS BALANCE AND FELL ABOUT 3 1/2' ONTO THE CONCRETE FLOOR.
EE GOT DOWN FROM CAT TRUCK TO CHECK FOR A FLAT. THE TIRE WAS OK & HE WAS GOING TO RETURN TO THE TRUCK. HE SLIPPED ON THE SNOW & FELL. HE TRIED TO GRAB THE HANDRAIL OF THE TRUCK BUT FELL ANYWA Y. HE PULLED MUSCLES & LIGAMENTS IN HIS SHOULDER
HE WAS GOING THROUGH THE TUNNEL PASSING UNDER THE FIRST FEEDER TO REPAIR THE SECOND FEEDER. A ROCK FELL OFF THE FIRST FEEDER AND HIT HIM IN THE HEAD.
EMPLOYEE WAS WALKING TOWARD THE CRUSHER SECONDARY BOOTH UNDERNEATH THE 8X20 VIBRATOR AND HE STEPPED ON A LARGE ROCK CAUSING HIM TO TWIST HIS LEFT ANKLE AND FALL.
WHILE PASSING CONVEYOR CAUGHT HAND BETWEEN BELT AND ROLL.
