Individual was unloading a trailer when the scoop he was operating slid off the trailer causing him to strike his head against the canopy receiving a laceration to the head.
EE was scooping the rib on roadway at the overcast and a roof bolt that was buried in gob, caught scoop bucket and flew back and caught him over the right eye causing a laceration. EE was wearing his safety glasses; the roof bolt broke his safety glasses.
While loading a shuttle car it shoved the boom of the miner into the top and sheared a roof bolt off which came down and struck the injured on the corner of his eye.
The employee was operating a Cat 777 rock truck headed to the dump site when he hit a bump and was jarred injuring his lower back.
Employee was loading binder on coalburg seam. When attempting to load a bucket of material the bucket caught a hard spot that stopped the loader, jarring his whole body especially his back. He contacted the Foreman and denied transportation to the ER. Was X-rayed and told nothing was broke, He started missing work on the 28th and ruled compensatory on March 14.
The injured was driving on the Bearpen haulroad and as he started to negotiate a 3-4 percent grade and curve at road marker #18, his pickup truck slid into a loaded coal truck. Due to the potential serious injuries sustained to his head and chest areas, he was air lifted to a local hospital; he was released on 12/31/2010. The coal truck driver sustained first aid injuries.
Employee states that as he was attempting to dump a load of coal from trailer, the coal stuck in trailer causing the front wheels of tractor to raise off the ground. When the coal dislodged, the trailer came down suddenly, jarring employee.
Employee said he was sick, had M.E.T. check him out, his B/P was 190/110. He was transported to ER. After arriving at ER employee said that while pulling cap off of Buffalo seam he caught a hard spot that jarred his neck and back. He worked for a time and the pain and stiffness continued until he got sick and vomit. X-ray was negative and Dr. took him off work starting 12/17/2010.
While traveling down the #5 entry toward #2 section, the rubber tired mantrip EE was operating, hit a rut and caused the steering wheel knob to strike EE's right arm.
While trouble shooting the ram on a hauler duster, the injured was looking up under the ram, his foot was down in a cleaning hole in the false bottom of the hauler, he had the operator pull the ram back which closed in on his foot.
While unloading hi-line from a car 10/11/2010 caught the two small toes of his right foot under a wheel. He continued working not seeking medical attention until 10/13/2010 after swelling and pain persisted. X-rays did not reveal any fractures and he was released to unrestricted duty. Became a Restricted Duty after the site of initial injury became infected 10/27/2010.

Employee states he became nauseous from fumes upon inspection of battery box during the truck's daily service in shop. "REPORT BEING FILED UNDER PROTEST"
Employee was hauling coal from the miner to the feeder when the shuttle car jarred employee. Employee finished shift and reported soreness in lower part of back.
Injured was operating a scoop. The scoop went over a ledge and jarred the operator. He informed his foreman of incident and worked the rest of the shift. Called off work the next day complaining of pain in his lower back. Placed off work by a doctor.
Coal Hauler approached continuous miner to load. Hauler struck operator in leg and operator sustained fracture to lower right leg.
The employee hit a bump in the roadway and struck his head on the canopy of the machine. The employee subsequently resigned on 3/15/2011.
EE was operating a 240 Ton Terex truck getting loaded when a rock slid off the bucket of the shovel causing a sudden jar which resulted in a pain in Employee's neck.
While operating a mantrip struck a battery ramcar on the travelway. The impact caused him to strike his head leaving a laceration. EMTs placed him on a backboard and transported by ambulance to Springfield Memorial ER. Treatment included staples to close the wound. A CT Scan revealed transverse process fractures. He was admitted for observation and discharged 2/24/2010.
While operating the scraper, hit a hole or bump in the road, causing him to bounce in the seat and causing a strong pain in his lower back. X-rays and a CT scan revealed a Vertebral Compression Fracture and he was directed to return to work and placed on desk work only restriction.
EE was hauling shot rock in a 785B Caterpillar Rock truck and as he was backing up to dump area his right rear tires set down causing the truck to overturn.The truck came to rest approximately 174 feet down in the hollow fill.
In #5 entry at spad #839 EE was pulling a loose piece of rock off the right corner. The rock come down and pulled the hung miner cable down onto his head, causing pain in lower back.
While recovering high line and lacing it onto a supply car his left foot was run over by the car pinching his big toe causing a fracture and leaving a laceration that required sutures to close. Treated at St. John's Hospital ER and released to unrestricted duty.
Injured was building stopping when a coal hauler came through the curtain and knocked stopping down onto injured. Injured's right knee hyper-extended.
EE was operating a shuttle car when he ran over some coal on the roadway causing him to slide into the coal rib. Sprained his lower back.
Employee notified foreman about 8:30 pm on July 14, 2009 that his back had started to hurt. He asked the foreman to fill out accident report Jarring of back from truck was mentioned He was offered a ride to the ER but declined. He went to his personal Dr. the next day and was excused from work. He returned the 17th but left after 4 hrs. 8/3/09 accident determined to be chargeable.
The employee was operating a 2 person personell carrier, when he ran over some gob in the roadway. The steering wheel turned sharply, bending his right hand back, breaking his ring finger on right hand.
The employee was operating a battery mantrip when he leaned up hitting his head on an overcast.
Injured stated that as he started to tram a 2-man Johnson Industries battery ride, it went into high tram unexpectedly. Bounced over a few ruts in roadway and was jarred off ride. Landed on right lower pelvic area.
While backing little two man Johnson buggy up, ran over loose coal and got head in the top and stove neck up. Did not have any medical treatment til 5/5/09 and started missing work on 5/6/09.
A blast was detonated causing material to slough off the outside of a hillside near the shot. The material displaced mud from an existing pond creating a wave of mud. EE's truck slid down an embankment, causing a contusion to his head.
While driving a mantrip he hit a bump or a hole in the travelway & jarred his back causing immediate pain. Drove himself to Hospital ER in Lincoln where a diagnosis was inconclusive & pain medication was prescribed. An additional follow up with Bansal Occupational in Springfield put him on a weight & position restriction & 2 weeks therapy for a lumbar strain.
EE was going to the continuous miner to get a load of coal, when he struck the coal rib with the shuttle car he was operating. He struck the canopy with his head, springing his neck.
Employee had positioned his left leg over the pusher assembly trough in order to add grease to a belt alignment fitting. The pusher assembly flipper was advanced and his leg was pinched between the flipper and a conveyor car pusher block resulting into a fracture and muscle damage to his left leg.
He was moving shuttle car for the continuous miner to travel by, when his head struck a low corner. He was taken to the ER and checked out and released that night with strained muscles in the neck and head area.
While driving a water car and looking to see if the sprays were working he turned forward and had driven into a splice box striking his left arm, face and chest. Transported by ambulance to St. John's ER where initial diagnosis revealed a cracked sternum, fractured ribs and a fractured left wrist. Admitted for observation expecting to be released 1/12/2009.
While travelling on a golf cart he grabbed a Heitzman steel beam to stretch himself, the beam fell knocking him off the cart, lacerating his right hand and straining his upper body. Transported by ambulance to Memorial ER where CAT-scan and X-rays revealed no fractures and the laceration was closed by suturing. He was directed to follow up with his personal physician.
While standing next to the refuse bin a chunk of refuse bounced off the scraper and struck him on the left ankle. Transported by private vehicle to St. John's ER and released with a `weight bearing as tolerable' restriction. He began missing work 10/8/2008 due to pain. A follow up visit to Midwest Occupational Health Associates (MOHA) is scheduled for 10/15/2008.
EE was operating a scoop, ran over a rock in the road way and EE struck his head and shoulders on underside of canopy. Accident occurred in #5 entry between spad 279 and 286 along roadway on 1st right.
Employee was operating miner in 5 rt x-cut when a coal hauler struck employee on the right ankle. Employee was treated & released to return to work. He experienced continued pain & swelling. Follow up visit with specialist on 10/20/08 revealed torn tendon in foot. This became lost time & reportable on 10/20/08.
The injured, along with 2 others, were installing a mechanical splice in a piece of mining belt when a scoop struck them.
Employee stated he ran over a rock in the roadway causing the coal hauler to bounce. His hat bounced off and then he struck his head on the underside of the hauler canopy.
Standing between roof bolter and 2 man buggy in break between #6#7 entry. A ram car came around block in #6 entry going to Continuous miner hitting 2 man buggy and pushing it into the roof bolter, catching his right leg between the two pieces of equipment. Started missing work on 9/12/08.
EE was working in a new shot making a loading pad. The material was on a different level than the previous shot. In order to move material, he was running the loader bucket into solid rock. On one of the passes he felt pain in stomach. The pain increased until he called the foreman and transported to ER. CAT scan found hernia. Started missing work on 7/16/08, history of hernias.
Employee raised left hand to keep boom from powder truck from hitting in the face. They were loading holes to be shot. the boom came in contact with the outside part of left hand causing injury. He finish his shift and continued to work until Thursday June 5, 2008 before going to ER.
EE was in the process of hanging a block curtain across the belt out by the feeder when a belt splice caught the curtain and jerked him down resulting in a fracture to his left ankle.
While scooping the feeder, the rails on the operators side of the batteries slide forward into the cage around the operator. When this occurred, the operator had his left hand holding onto the cage, which exposed the fingers to the outside of the cage. As the rails struck the cage, the end of one amputated the tip of his left middle finger
