Haul truck operator hit a couple of potholes and tensed and applied brakes while traveling haul road. Soon after the operator saw a truck sliding towards them, so they tensed up and applied brakes again. In tensing up, the operator felt pain in left shoulder. The operator ran two more loads and, on the way, to tie down, the pain increased.
While making a turn on SP11 dump in HT 62, the employee bumped their right elbow on the Haul truck steering wheel spoke and caused swelling and right contusion
While cleaning P92A ore pass grizzly in P91 at around 21:25 of plug reversing in 1st gear, hit rib with left rear corner of loader. Upon contact with rib, low speed, felt sharp pain in front right shoulder.
Operator was mounting a Haul Truck and raising the boarding ladder. The operator's hand was on the boarding ladder switch when the handrail to the ladder pinched their finger between the switch and the handrail.
The haul truck operator had completed dumping and was traveling back down the haul road. While driving, the operator struck a pothole at approximately 13 miles per hour. As a result of the impact, the operator reported experiencing lower back pain. Operator went restricted duty on 3/9/2026.
Employee experienced eye irritation from an unknown particle and attempted to flush their eye; later in the shift the irritation persisted and the employee was seen for an eye abrasion.

Employee pinched their right pinky finger between the forklift mast and the tip dumpster while assisting another employee in latching the tip dumpster. A fracture was diagnosed on 02/24/2026.

While attempting to make a left turn in the company LV335 flatbed truck, it was hit by a personal vehicle on the driver side door. The impact caused the employee to hit their head on the driver side window.

Employee struck driver's side door of company vehicle with personal vehicle. Front air bag deployed. Employee has reported neck pain on 2/24/26. Examined at Health clinic on 2/25/26.
The operator of haul truck claimed that while dumping out the front end of the truck lifted up and slammed back down. They stated at the time of incident they did not experience discomfort. However, as the shift progressed, they began to experience increasing discomfort to their lower back. The operator went on leave for the injury on 1/20/2026.
The employee was leaving EE's Shovel 6 assignment when EE made contact with a pothole in the haul road. The Employee states EE has strained EE's lower back pain.
The employee was leaving Shovel 4 and 6 pit in 6th gear when EE hit a pothole in the road. The employee stated the truck bed raised up a little and came down, causing the seat to bottom out. The employee was seen by Occ Med on 9/23/25 and was released without restrictions. On 9/26/25, the employee was seen by Occ Med and was returned to work with restrictions.
While removing a catwalk by the plenum, a forklift fork became caught under the platform and lifted it slightly, grazing a welder's shin and causing a small cut above the left ankle.

On 3/3/2026 claimed to be injured from an incident where they aggravated a per-existing injury on 9/9/2025. After investigation the company administered workers compensation. Employee lost time after injury was reported.
After getting a bucket of material, the loader loading the haul truck backed up and turned too quickly causing the bucket of the loader to make contact with the truck bed. This caused the haulage operator to suffer a back muscle strain. Case went recordable on 09/13/2025 when the operator was given restrictions.
Rubber tire driver hit a few potholes and felt a slight pain on EE's chest and low back. When the driver stopped to check equipment, EE felt dizzy and could not stand.

The employee was in the cab of the radial stacker when the 2450-002 luffing system base failed at end of shift causing the stacker cab to drop to the right side. The operator in the cab, experienced some discomfort to the left leg after the luffing system failed.
While disconnecting the rear hose attachment of the CCV003 (Vac Truck) to clear a plugged hose, the top part of the hose whipped back and hit the employee's front tooth, cracking the bottom part of the right front incisor (central) tooth.

While driving through SH3 pit, a haul truck trainee employee hit a rough spot in the road. Initially the employee stated they were ok but after another run, they told their mentor that they were experiencing back pain.
Haul truck dumped out and was traveling to the next assignment. As EE headed down the Power ramp, EE's truck began to slide due to the roads being slick from the weather event just before the shift. Driver tried to correct the slide, and EE made contact with the berm with the right front corner.
Employee was sitting in the driver's seat of the employee's GMC 3500 when the truck was struck by a forklift operated by an FMI employee. The FMI employee was picking up wall liners with a Teloiza extend fork PIT. Minor damage was caused to the GMC Truck.
