Miner operating a UTV too close behind a haul truck. The haul truck stopped and reversed direction. The bed of the truck struck the windshield and ROPS of the UTV. Miner injured right arm during the collision.
Miner was operating the sunbelt rental forklift working in the lay down yard area, got off to go inspect the work area and as miner went back to get on the forklift, placed one hand behind the seat area and when miner did that felt a sharp slice (cut) on finger.
Two electricians were working from a manlift to hang power cables from the roof. The electrician operating the manlift inadvertently swung the boom too close to the rib of salt and pinched the employee's hand between the basket and the salt.

Haul truck operator drove over a section of uneven ground and was not wearing a seat belt. The operator bounced up and hit head on the roof and hit knee on the dash. Operator was taken to the hospital for observation. Operator has been placed on modified duty.
