Employee was operating a scoop when the scoop slid sideways. The operator's deck on the scoop slid under a braw, the employee was pushed down in the operator's deck by the braw.
While pushing a miner drum out of the scoop bucket, the employee stated that EE laid EE's arm on top of the back rest and as the scoop drifted back, EE's arm slipped of the back rest allowing EE's arm to get caught between the backrest and the coal rib.
Employee was operating a scoop and ran over a rock. This caused the employee to be jarred inside the operating deck of the scoop.
Employee was tramming the roof bolter past a parked shuttle car. The bolter slid and caught employee's leg between the bolter and the shuttle car bumper.

Employee assisting to unload roof bolts off a flatbed truck from Fairmont Supply Company. Employee gave truck operator signal to lift, and unexpectedly the arm of boom swung out striking employee and knocking him off the truck. Causing injury to ankle and wrist.
Employee stated that while operating a shuttle car, he got his foot caught and twisted his ankle.
Man trip mistakenly was in forward direction instead of reverse. Man injured had back turned to man trip, operator of mantrip trammed forward and rolled into injured person, trapping him against pin machine.
The 3 wheeler mantrip operator lost a wheel bearing, causing the brakes to fail. When operator jumped off, he bruised his left shoulder & broke (1) rib bone.
Employee got out of his truck to talk to another driver. The truck he got out of started drifting. The employee jumped on the side of the cab of the truck and was partially inside the cab. This truck rolled alongside of another parked truck pinning his right leg between the door and the cab.
Belt man was traveling #2 haulway, lost his footing on brake pedal and slid in to the rib brushing chest area; still finished his shift.

THE INJURED EE WAS WALKING FROM THE POWER CENTER ENTRY TO CHECK THE DISCHARGE END OF THE FEEDER. WHEN HE WALKED ALONG THE EDGE OF THE FEEDER, HE LOST HIS FOOTING AND THE CONVEYOR CHAIN CAUGHT HIS LEFT FOOT DRAGGING HIM ONTO THE FEEDER. THE FOREMAN WAS ABLE TO TO STOOP THE FEEDER.
