A 988 loader was backing off of a feeder ramp when it struck another 988 loader which was waiting for other mobile equipment in the area to clear. The impact caused the operator of the stationary loader to strike his forehead on part of the rollover protection which in turn resulted in a laceration requiring stitches.

Showing all 35The employee finished his shift (owl) and was on the haul road going home. The coal trucks were on the same road going to the mines. The two vehicles collided and mine employee's car was hit on the driver's side by the bumper of the coal truck.
Employee was in scoop bucket getting ready to ride to face area. He had hand on top of ram scoop. Operator got in scoop and activated ram lever not knowing injured had hand in pinch point location.
The employee was positioned at the rear of the section feeder observing a stalled feeder chain. A parked coal hauler rolled down a grade, catching the EE between feeder & hauler resulting in injuries requiring amputation of right foot & ankle & fracturing the left leg. The accident is still under investigation.
Driver was instructed what stockpile to dump was in. When driver backed into position to dump. He informed loader op. He was not level enough to dump. Driver pulled forward a few feet and loader op. told him to stop there and dump and the loader op. would spot him (put bucket beside trailer) loader op. alledgedly failed to spot truck and it overturned.
Was riding rail runner to section. Got cramp in leg. Raised up, too high. Hit mine roof. Knocked him back down in rail runner, he hit frame of rail runner with left hip.
Employee was taking a coal sample from a moving belt. Shovel struck in belt causing right hand to strike metal tunnel cover.
Miner foreman crawled out into intersection to look, swag was in area, ram car came over swag. Foremen did not see car. The car artickiled down, bucket end hit foreman's right leg.
Injured employee was in the process of dumping a load of coal off of his tractor/trailer when the trailer turned over, which in turn turned the tractor over. Injured employee's neck was lacerated during turn over. What caused the laceration was undetermined.
Employee was cleaning coal in pit, using a 988G Cat loader, when the rear of the loader bumped the wall.

EE OF MACK RD800 SHIFTING GEARS. TRUCK DID NOT GO COMPLETELY IN GEAR. EE HAD ENGINE BRAKE ON, WHEN APPLIED BRAKE, ENGINE STALLED OUT, LOST CONTROL, ROLLED ONTO SIDE.
Employee came into foreman's office and reported that his back was hurting and that he was going to seek medical attention. He informed his foreman that he had injured his back while pushing up loads of coal in the yard.
Employee was backing shuttle car up with right leg hanging out of car. He caught his ankle between shuttle car deck and coal rib.
Employee was getting cable out from under a sled in the bucket of a scoop and the scoop driver dropped the bucket, catching the employee's top of his left foot between the sled and the bottom.
EE got out of his truck to clean his tailgate and did not set his parking brake. The truck began to roll, ee tried to get back in the truck, lost his balance and fell beside the truck which ended up on his lower legs. He had severe bruising and contusions to this area esp right leg.
Employee was cleaning vent on air conditioner and fingers came in contact with fan.

EE WAS SITTING ON TOP OF RAILRUNNER WHILE MOVING IT AND CAME TO A CURVE IN TRACK. THE RAILRUNNER JUMPED TRACK CAUSING EE TO INJURE LEFT SHOULDER. (STARTED MISSING WORK 10/25/04.

TRUCK A'S TAILGATE FROZE SHUT AND HIS LOAD DID NOT COME OUT, HE PULLED FORWARD, THE TRAILOR ROCKED AND THEN THE BOTTOM JACK PIN BROKE AND THE TRAILOR FELL OVER ONTO TRUCK B'S DRIVERS SIDE. THE TRAILOR TRAPPED INJUREDS FOOT ONTO THE FLOOR BOARD.
FOREIGN MATERIAL WAS BLOWN BEHIND EE'S SAFETY GLASSES INTO HIS RT. EYE. AS HE STARTED A BELT DRIVE. HE DID NOT MISS ANY WORK DUE TO THE INJURY, BUT PRESCRIPTION MEDICATION WAS USED. NO MATE RIAL WAS IN HIS EYE WHEN HE SAW A DR.
AS TRUCK WAS DUMPING IT TURNED OVER ON IT'S SIDE. DRIVER HEARD A BANG THEN TRUCK TURNED OVER.
EE WAS OPERATING A SHUTTLE CAR. AS HE WAS TRAVELING UP THE ENTRY, HE PLACED HIS HAND ON TOP OF THE CAR CANOPY AND HIS FINGER WAS CAUGHT BETWEEN THE CANOPY AND THE MINE ROOF. FRACTURE AND LACER ATION TO LEFT RING FINGER. RETURNED TO REGULAR DUTIES NEXT SCHEDULED SHIFT.
