Telehandler operator picked up a bundle of 20' x 6" c-channel. The c-channel began to slide from the end of the forks toward the mass of the telehandler. The IE, who was near the telehandler, attempted to prevent the channel from sliding back on the forks resulting in a laceration that required 5 stitches to close a wound on EE's middle finger.

An employee was operating a mill loader to run rock from the mine stacker pile to the mill. They were on the west side of the pile to grab a bucket of fines; the employee then began to back up and came to an abrupt stop (loader backed into the radial stacker). They then turned around to look and saw that the radial stacker was coming down on top of the loader.

Operator explained R knee felt irritated due to making contact with panel on right side of loader cab while operating the machine throughout the entire shift. Dr. said this did not explain the swelling in knee and diagnosed employee with right knee pain. Employee released to desk work.

Employee was driving 06 Haul Truck down Hokie Pit Road, where other equipment had been stuck in the mud on the previous day. There was ruts in road where dozer had turned to pull out stuck equipment. Employee hit ruts, causing employee to bounce and seat suspension bottomed out, jarring employee's back. Employee was taken to clinic and released.

Operator just dumped a load in the dump area from the CAT 657G scraper. As they were leaving the dump area, they drove through an area of roughness as they were lowering the apron. The rough ground jarred the scraper, causing the operator's left shoulder to strike the safety bar on the left side of the operator's cab causing a fracture to left clavicle.

EE was operating the loader, the air conditioner did not work so EE opened up the windows for air. At 2:00 pm left eye was irritated. In the morning when EE woke up the eye was almost swollen shut. EE was taken to doctor (5/27/16) where the doctor did not find anything in the eye. The doctor diagnosed as allergy, gave employee steroid eye drops and released employee to work.

Employee was attempting to step up onto the rear step of the powder wagon. As he did, his foot slipped and he felt a slight sensation in his left hip area. The slip was contributed to mud on the step and worn soles on the employees safety shoes. An internal audit discovered this was not recognized as "reportable" until 4/9/2012.

Miner (mechanic) was directing operator of forklift in placement of boxes of dust collector bags as part of dust collector work. Miner did not pay attention to hand placement in close proximity to forklift tines. Forklift tine contacted miner's left index finger causing an avulsion on top of left index finger. Miner received stitches to close the avulsion of the left index finger.

EE WAS BACKING DOWN A RAMP ON A WORKSAVER. THE WORKSAVER BECAME UNSTABLE & HE STRUCK A PARKED FORK TRUCK. HIS RIGHT LEG WAS OUTSIDE THE WORKSAVER & WAS PINCHED BETWEEN IT & THE PARKED FORK TRU CK. DUE TO THE UNSTABLENESS OF THE WORKSAVER, HE LOST HIS BALANCE, CAUSING HIS RIGHT LEG TO BE OUT OF THE WORKSAVER DURING THE COLLISION.

THE EE WAS OPERATING A CATERPILLAR 777 HAULAGE TRUCK AND WAS HAULING OVERBURDEN. WHILE TURNING AROUND AT THE DUMP SITE, THE TRUCK BROKE THROUGH THE FROST CAUSING THE TIRES TO SINK INTO THE GRO UND AND TIPPING THE TRUCK ON ITS SIDE. THE IMPACT CAUSED A SPRAIN TO THE EE'S LEFT SHOULDER REQUIRING PRESCRIPTION MEDICATION.
