EE and his foreman were adjusting the drop point of the out conveyor about 6" so that the material would fall on the conveyor of the belt. The bucket of the skid steer slipped from conveyor & pinned EE ankle to the ground resulting in fracture lower left leg.
EE was too close to chain conveyor as loader pulled back, pinching finger.
Was driving in a rough terrain, air ride seat bottomed out, causing pain in lower back.
Was driving water truck downhill, missed a gear and rolled it.
CAT DOZER WAS STUCK IN SOFT WET GROUND. TRIED TO PULL OUT DOZER WITH LOADER. CHAIN BROKE SHATTERING BACK WINDOW GETTING SMALL CUTS TO RIGHT SIDE OF FACE & FRAGMENT IN EYE
EE WAS OPERATING THE SKID STEER WITH HIS LT.FOOT RESTING OUT THE FRONT OF THE OPERATOR COMPARTMENT, OPERATOR LOWERED FORKS & PINCHED FOOT, BREAKING HIS FOOT.
EMPLOYEES WERE CLEANING OUT TAIL PULLEY MATERIAL(CONVEYOR LOCKED OUT) BUT MATERIAL ON BELT CAUSED BELT TO ROLL BACKWARDS AND PINCH EE'S HAND IN PULLEY. HIS LEFT HAND HAS 2 BROKEN BONES IN FING ERS AND BRUISED MUSCLES.
EMPLOYEES WERE MOVING STACKER WHEELS TO PIN FOR RADIAL DRIVE & WHEELS SEEMED STUCK WHEN WHEELS FREED UP THE EMPLOYEES HAND WAS PINCHED BETWEEN FRAME AND SUB-FRAME.
