INJURED EE WAS WORKING ON M-55 SCREW CONVEYOR TRYING TO RESOLVE A FEED PROBLEM TO THE PACKER THIS AUGER FEEDS. HE WANTED TO VERIFY THAT FEED WASGETTING TO THE AUGER, AND REMOVED THE CAP ON THE BOTTOM OF THE AUGER. USING THE HOA SWITCH AS HISISOLATION OF ENERGY, HE STATED THAT "HE THOUGHT HE HAD IT OFF" BUT IT WAS IN AUTOMATIC. HE PLACED HIS HAND INTO THE MOVING AUGER.
EE WAS OPERATING A PORTABLE LIFT AND WHILE BACKING UP THE LIFT HE GOT HIS RT. THUMB PINCHED BETWEEN THE LIFT HANDLE AND A PALLET. HE RECEIVED FIRST AID BY THE COMP. NURSE FOR SWELLING & BRUISI NG OF THE HAND AND RETURNED TO RESTRICTED DUTY.
EMPLOYEE LOST RIGHT MIDDLE FINGER TIP WHEN HIS GLOVED HAND CAME IN CONTACT WITH AN OPERATING ROTARY AIRLOCK FEEDER. HE WAS ATTEMPTING TO CLEAN A CLAY POWDER BLOCKAGE AT A CLEAN OUT PORT.
EE WAS DRIVING FORKLIFT IN LOADING ROOM WHEN A FOREIGN PARTICLE ENTERED HIS EYE RIGHT ALTHOUGH HE THOROUGHLY FLUSHED HIS EYE WITH WATER, THE EYE WAS STILL IRRITATED THE NEXT DAY. HE WAS SENT T O THE HOSPITAL WHERE HE WAS DIAGNOSED WITH DEBRIIN THE EYE AND SCLERAL ABRASION. ANTIBIOTIC OINTMENT WAS PRESCRIBED TO PREVENT INFECTION.
