Mechanic was performing maintenance when a piece of metal moved and injured EE's arm.

Struck against stationary object

EE was trying to get the handle to slide shut on the back door of the container. It was not closing so EE was hitting the handle with right hand. After a few times, EE's hand missed the handle and hit the container.

Struck by powered moving object

An over the road truck driver dropping off a load was struck by a golf cart after they had exited their vehicle. The driver indicated that they were not injured but later reported pain and went to a doctor. Medication was prescribed with two days out from work from the doctor.

Contact with electrical current

While pumping product out of a lined container, the friction from the product flowing through the hose, shocked our employee, knocking EE to the ground. EE never lost consciousness.

Over-exertion in pulling or pushing objects

EE was prepping containers, when EE grabbed a liner bag off of a stack of liner bags to pull the bag to forks of the forklift, the wooden part of the liner caught the side of the door frame and pulled EE back causing a pain in EE's right shoulder.

Struck by flying object

The employee was gouging out old weld, EE dropped the air arc lead and the button actuated when it fell to the floor. The blowing material went under EE's safety glasses and irritated EE's eye. Particles on the floor and dropping the air arc lead were contributing factors, the employee was using correct PPE.

Over-exertion in pulling or pushing objects

Employee was working on 11A paper bagger when bag became jammed at discharge of bag flattener. Employee attempted to pull bag free. Employee suffered strained muscle in upper back/neck area.

Struck by rolling or sliding object

Employee was attempting to get slide gate to activate. Slide gate opened striking employee on right forearm producing cut that needed 4 sutures to close. Employee returned to work same day on restricted duty.

Struck by flying object

There was an incident where a driver was driving truck when they met IE driving IE's truck. EE swerved to EE's right causing EE's arm of EE's tailgate to strike IE's side mirror, which made the mirror break IE's side glass. It caused a small cut to IE's face. All involved were checked out at BASF nurse & had a drug test at the Urgent Care.

Brick layer was assisting to remove refractory debris from the kiln. They reached for a piece of debris and a second piece of debris shifted onto the 5th finger, pinching the tip. Employee was wearing full face respirator, hard hat, steel toe shoes and leather gloves.

Fall to the walkway or working surface

Employee tripped and fell on same level. Reported bruising to right rib cage area on back. Received first aid treatment. Several days later seen by physician. X-rays of area were negative. Returned to normal duty that day. No lost work days, No restricted work days. Prescription muscle relaxer was prescribed to assist with pain.

Installing 11 gage stainless steel plate on 11G spray driver. While lowering the plate through the scaffolds, plate hung up on the 2nd or 3rd scaffold, plate shifted and hit employee.

Packaging Operator was struck on palm of right hand by B4 bulk bagger arm when the bolts holding the bulk bagger arm broke allowing bagger arm to move in downward motion. Operator received a puncture wound to palm of right hand requiring 6 stitches. Employee reported back to work without lost time on restricted duty.

Mechanic received a crush injury to right thumb resulting in amputation of right thumb nail bed to tip of thumb. Mechanic was lowering a 350 hp electric motor to motor base utilizing an electric crane hoist and had thumb positioned in the ""line of fire."" Electric crane performed as designed.

The employee was working on the catwalk on top of a silo. He states that he caught his foot causing him to fall forward and hit his knee on the mid-rail of the catwalk. No one on the crew witnessed or heard anything.

While walking up an embankment, employee slipped and fell backwards, hitting himself in the face with surveying equipment - causing 8 stitches to repair a 3 cm gash in lower lip/chin.

Employee was cutting and removing straps from a container he was preparing to load. During the process of cutting a strap, the knife made the cut and continued towards the employee's face, striking him below his safety glasses and right eye.

A Mechanic suffered a sprained right knee after slipping and falling. While performing an equipment inspection, the mechanic stepped over a 12"" high concrete retaining wall. The area behind the retaining wall was very slippery due to heavy rain combined with kaolin dust. As the mechanic stepped over the retaining wall, he lost his footing and slipped resulting in a fall.

Employee pumped liquid kaolin into a tanker trailer without venting the trailer. The trailer consequently pressurized itself. When he attempted to vent the trailer while unloading, he unscrewed a lid on the top of the trailer and the pressure inside the trailer caused the lid to pop off and hit him in the nose. Sustained a laceration to the nose.

Employee was using a car stop as a hammer to move a rail car door slide back onto the slide rail. When the employee struck the door slide, the car stop ricocheted and hit the employee's safety glasses, resulting in a laceration under right eye on right cheek, which required 5 stitches.

Employee was attempting to catch a sample on top of a silo when his left middle finger contacted a screw conveyor resulting in an avulsion which required sutures.

Employee sustained a right eye injury which he believes occurred while working in the vicinity of an employee who was welding. However, employee was diagnosed with a corneal foreign body and thinks he may have encountered some trash that blew into his right eye while working around calciners.

Ee was repairing a broken cable on bulk loading chute. When he went to lower the chute, he hit the wrong button and raised the chute instead. His right hand was caught in the cable resulting in laceration and fracture to the right index finger. He received treatment at ER.

A LAB OPERATOR WAS OPERATING A MIXER IN THE LAB. WHEN HE WENT TO REMOVE THE CUP FROM THE MIXER, THE MIXER CAUGHT HIS RUBBER GLOVE AND CUT HIS LEFT MIDDLE FINGER.

MECHANIC WAS HELPING REMOVE INPELLAR FROM PUMP. A 48 INCH PIPE WRENCH WAS HELPING TO HALF SHAFT . CHAIN HOIST WAS HOOKED TO IMPELLAR, TO REMOVE IT. PIPE WRENCH BROKE AND PART OF WRENCH HIT EE ON RIGHT SIDE OF FACE.

EMPLOYEE WAS ATTEMPTING TO EXIT PULVERIZER ROOM AT LAB WHEN THE DOOR HANDLE JAMMED. HE STATES THE ROOM BECAME VERY HOT SO HE USED A PULVERIZER METAL/STEEL CUP TO BREAK THE DOOR WINDOW AND SUST AINED MULTIPLE LACERATIONS AND CUTS TO HIS RIGHT HAND.

EE RECEIVED CHEMCIAL BURNS ON FACE, CHEST AND ARMS WHEN HE DISCONNECT A HOSE THAT WAS UNDER PRESSURE AND THE CHEMICAL SODIUM SILICATE SPLASHED ON HIM. THE SDS PUMP ON THE CUSTOMERS TANK WAS HU NG UP AND NOT ALLOWING THE PRODUCT TO MOVE FROM THE TRUCK/TRAILER INTO THE SDS TANK SO DRIVER DISCONNECTED HOSES NOT REALIZING PRODUCT WAS STILL IN HOSE.

MAINTENANCE MECHANIC WAS IN PROCESS OF MOVING A FURNACE INTO FIRING POSITION. AS HE WAS INSTALLING BOLTS, HE LOST HIS FOOTING AND FELL 18' FROM THE FURNACE PLATFORM TO THE GROUND. HE WAS TRANS PORTED TO E.R. BY AMBULANCE. HE SUSTAINED MULTIPULE FRACTURES AND IS STILL HOSPITALIZED, AWAITING SURGERY.

EE WAS INSPECTING COOLER COLLECTORS. A LARGE GUST OF WIND BLEW THE DOOR OF THE COLLECTOR CLOSED THEY HAD BEEN HELD OPEN BY DOOR STOPS. THE DOOR STRUCK EE LEFT ARM AS IT SWUNG CLOSED. INITIALLY DIAGNOSED AS BRUISED WRIST FOREARM BUT LATER X RAY SHOWED FRACTURE OF ULNA. EE PLACED ON LIGHT DUTY.

EMPLOYEE WAS ATTEMPTING TO CHECK THE PUMP AT THE BOTTOM OF 102 TANK. AS A RESULT OF THE RAINY WEATHER THE ENVIRONMENT WAS WET AND SLIPPERY. WHEN THE WORKER STEPPED INTO THE PIT AREA SHE SLIPPE D, FELL AND SPRAINED HER ANKLE.

TWO MECHANICS WERE INSPECTING THE TUBULATOR AT 11F HIGH BULK PLANT DURING DOWN TIME PERIOD. AS PANT OF THE INSPECTION, ONE MECHANIC, INJURED EE SPUN THE TURBULATOR TO MAKE SURE THE BLADES WERE NOT CONTACTING THE HOUSING IT ROTATED WHEN HIS LEFT THUMB WAS CAUGHT BETWEEN THE HOUSING AND ONE OF THE BLADES AND PARTIALLY ALVUSED IT.

EE WAS DRIVING FORKLIFT IN LOADING ROOM WHEN A FOREIGN PARTICLE ENTERED HIS EYE RIGHT ALTHOUGH HE THOROUGHLY FLUSHED HIS EYE WITH WATER, THE EYE WAS STILL IRRITATED THE NEXT DAY. HE WAS SENT T O THE HOSPITAL WHERE HE WAS DIAGNOSED WITH DEBRIIN THE EYE AND SCLERAL ABRASION. ANTIBIOTIC OINTMENT WAS PRESCRIBED TO PREVENT INFECTION.

MAINTENANCE WAS IN THE PROCESS OF REPLACING BOTH PADDLE MIXERS SHAFTS AND PLANT 17. A HYDM LIFT WAS BEING USED TO LIFT ONE OF THE NEW SHAFTS INTO PLACE. WHEN THE LOAD SHIFTED A MECHANIC, WHO W AS HELPING TO GUIDE IT INTO PLACE, FELT A PULL IN HIS LEFT FOREARM.

TWO EMPLOYEES WERE ENGAGED IN DROPPING AN EMPTY HOPPER DOWN TRACK 5. AS TACKMOBILE APPROACHED, THE BULK LOADER REMOVED THE RAIL AND RELEASED THE BRAKES ON THE RAIL CAR. THE RAIL CAR COLLIDED W ITH THE TRACKMOBILE SOME 35 PACES DOWNGRADE FROMITS ORIGINAL POSITION. AT IMPACT THE OPERATOR'S LEFT LEG STRUCK THE TRACKMOBILE STEERING COLUMN.
