Showing all 4Employee was driving a CAT 769 Haul Truck. Reached behind the seat to see if the cooling fan in the cab was running and stuck his finger inside the guard and cut tip of middle finger on right hand. Taken to hospital and stitched up.
EE was attempting to remove a rock that had become lodged under the belt in a conveyor tailpulley using his hands to roll the belt forward. The rock dropped out and belt rolled back, pinching both hands. The little finger on his left hand was broke & the tip above the first joint was badly cut. He was taken to the hospital and the little finger was stitched up.
QUARRY FOREMAN WAS WORKING UNDERNEATH HIS OWN PICKUP. HAD PUT FRONT WHEELS UP ON RAMP. HE LOOSED DRIVE SHAFT AND PICKUP ROLLED OVER THE CHEST CAUSING BRUISES & ACUT ACROSS HIS FOREHEAD.
