Employee was driving a CAT 769 Haul Truck. Reached behind the seat to see if the cooling fan in the cab was running and stuck his finger inside the guard and cut tip of middle finger on right hand. Taken to hospital and stitched up.

A flop gate (75 lbs) at the end of a conveyor had rolled up and landed on the tail pulley. Two employees raised it up to let up back in position. One employee let go of his end after the gate went over center before the other and the gate pinched the right ring finger of the second employee against the angle iron that the gate swung in.

EE was attempting to remove a rock that had become lodged under the belt in a conveyor tailpulley using his hands to roll the belt forward. The rock dropped out and belt rolled back, pinching both hands. The little finger on his left hand was broke & the tip above the first joint was badly cut. He was taken to the hospital and the little finger was stitched up.

EMPLOYEE WAS HELPIN PUT PUG MILL BACK TOGETHER WAS FINE UNTIL WEST HOME THAT NIGHT AND BACK STARTED TO STIFFEN UP. LIFTING DURING DAY AND CAUSED LOWER BACK STRAIN.
