As operator approached 5' to 6' pile, his loader was on an incline with bucket full of sand. As he put loader in reverse, a clump of material came off the top of the bucket and shattered the windshield. Loader tipped slightly backward and sand in bucket came into cab, crushing console and steering wheel. Employee sustained cut on right arm and fracture/bone chip in right ankle.

Backed overpile of material, felt jolt up through seat into his back. Started to feel pain early last night, reported at 10:00 this morning did not feel well and requested medical care.
While trying to clear the chute using a pry bar. EE lost his grip on the bar. The bar hit the belt-which was still running. The bar bounced back, hitting him in the chest and mid-section. Diagnosis at clinic is minor fracture of sternum.
Complained of neck and back pain; sent to medical center for evaluation. Originally not MSHA reportable as it was first aid care on 12/28. Employee was re-evaluated and referred to physical therapy.
Leaving mine in his personal vehicle. Side-swiped by pickup driven by another worker. Initially seen by dr. on 9/18/06 and released with no treatment rendered. Returned to dr. on 10/6/06 with complaint of pain. Returned to full duty. Medication prescribed.
As haul truck passed by, rock and dirt were kicked up, struck EE on the cheek and then the left eye. Advised by MSHA on 6/2/06 this is reportable since eye drops were dispensed as preventive measure.

Team member reported on 8/22/05 that she was experiencing increasing discomfort and pain in her left wrist progressing to numbness in left arm over 3 week period. Uses left hand to shift. Sought medical treatment on 8/23/05.
HAULING MATERIAL. WHEN THE LEFT FRONT TIRE OF THE SCRAPER HIT A SOFT SPOT, THE STEERING WHEEL SPUN ABRUPTLY TO THE LEFT. EE'S RIGHT THUMB JAMMED INTO THE STEERING WHEEL SPOKE. THIS OCCURRED CLOSE TO THE END OF EE'S SHIFT FOR THE DAY.
EMPLOYEE ADVISED COMPANY ON THURSDAY, AUGUST 26, 2004 THAT HE HAD BEEN TREATING WITH HIS PERSONAL CHIROPRACTOR FOR LOW BACK PAIN. THE ADVISED FELT THE PROBLEM WAS CAUSED BY OPERATING THE SCRAPER.
ON 7/6/04 DUST PARTICLES BLEW IN EYES WHILE OPERATING THE LOADER. EE REPORTED THE INCIDENT ON 7/7/04. SENT TO CLINIC/TREATED & RELEASED TO REG. WORK SAME DAY. NO LOST TIME OR WORK RESTRICTIONS INVOLVED.
EMPLOYEES WERE BACKING UP SLOWLY AND RAN INTO A BACKHOE. PASSENGER IN TRUCK COMPLAINED OF SHOULDER/BACK PAIN AND WAS TAKEN TO MEDICAL CLINIC.
AS EE WAS TRANSPORTING CONVEYOR BELT MATERIAL WITH FORKLIFT. THE BRAKES FAILED TO STOP THE FORKLIFT, EE RAN INTO AN EMBANKMENT. THE FORKLIFT CLIMBED THE EMBANKMENT, TIPPED AND SLID INTO THE CONVEYOR.
WATER TRUCK BECAME STUCK IN LOOSE MATERIAL. WHILE USING THE D-10 DOZER TO HELP IN MOVING THE TRUCK, THE WATER SHIFTED IN THE TANK, CAUSING THE TRUCK TO TIP.
WORKER REPORTED EXPERIENCING LOW BACK PAIN FOLLOWING REPETITIVE OPERATION OF SCRAPER.
FOREIGN BODY IN EYE. MUD TRUCK WAS DUMPING A VERY WET LOAD OF MUD. WHEN IT HIT THE GROUND, MUD HIT HIM IN THE FACE. MUD WASHED INTO HIS EYE.
THE LOADER DROPPED PART OF HIS LOAD IN FRONT OF THEW EUCLID. EE IMMEDIATELY APPLIED THE BRAKES WHICH CAUSED HIS NECK TO BE SNAPPED FORWARD, CAUSING CERVICAL STRAIN.
HIT A BUMP AND FELT PAIN IN HIS BACK.
