The employee was working on a bag packer palletizing 50 lb. bags of product. A damaged bag failed to transition from the belt conveyor to the skate rollers. The employee was attempting to remove the damaged bag from the conveyor system and inadvertently wedged fingers between the belt conveyor and the skate rollers causing abrasions to fingers.

Employee was driving a front-end load. EE was driving backwards in work area, the clay shed. While turning around to see where EE was going, hard hat fell off. The employee reached down to retrieve the hard hat and hit the back wall of the clay shed. EE's head struck the back glass of the loader.

Employee was preparing pallets for shipment in the #1 dock area. The forklift operator was moving pallets in the area. As the forklift operator was backing up, the forklift struck the effected employee in the left leg and ankle, resulting in a non-dislocated hairline fracture to the left tibia. Employee was treated by a doctor and returned to work.

The maintenance department was repairing a compressor located in the Mill Building. The crew had positioned the hub to install in the housing with a V45 CAT lift truck. The foreman was prying against the hub for better positioning when the fork assembly came off and fell across his hand.

EE operating a Volco L150C front-end loader scooped up a bucket of ore, drove to hopper. As he drove a 1' x 3' chunk of ore fell out of bucket, EE drove over the chunk causing the EE to bounce in cab of front-end loader, causing the neck injury.

EMPLOYEE WAS ADJUSTING CODE WHEEL ON THE POLYBAGGER WITH HIS LEFT HAND. HE PLACED HIS RIGHT HAND ON THE STATIONARY ROLLER. HIS HAND SLIPPED AND BECAME WEDGED IN A MOVING CONVEYOR BRUISING HIS HAND. HE DID NOT HAVE ANY BROKEN BONES. HE DID RECEIVE MEDICATION TO ELIMINATE THE PAIN.
