Over-exertion in lifting objects

The EE was in the bagger area stacking one load of 50lb bags on pallets. As they lifted their arm, they felt their right elbow go numb. Later in the day the EE started to feel pain in their right shoulder.

Employee sprained right ankle when getting off of forklift at propane tank.

Fall onto or against objects

Employee was cleaning out a chute with a shovel when EE slipped hitting EE's right side of EE's abdomen on a handrail. EE was seen 1 week later at doctor.

Employee received a second, conforming, B-read of EE's chest x-ray indicating a ""1/1"" degree of profusion in small opacities. Note: One of the B-readers noted that employee is a smoker.

Employee was using a pulley and chain when the chain Slipped and struck left thumb.

In mid-March employee was raising the roll up garage door when the door roll caught on the roll cover causing the chain to stop moving. The employee indicated that the left middle finger flexed when the chain stopped and was sore. On May 19,2022 employee indicated that the finger was still sore and was seen at doctors office.

Struck against stationary object

EE suffered a small cut on the right index finger while removing a bearing from a shaft on 02/22/22. On 03/31/22 employee reported some swelling and went the Doctor where a small sliver of metal was removed requiring 2 stitches and prescribed antibiotics.

Employee was entering the mill through the lab door. As employee exited, EE pushed on the door glass a little harder than EE should have causing the door glass to break. Employee was seen at ER and received seven stitches for two small cuts on the right hand.

Employee was removing hard hat to put on a safety harness. Employee caught the side of left eye with the hard hat's visor shield causing a scratch to eye.

Employee was removing a 50 lb. bag from the roller conveyor and cut employee's ring finger on employee's right hand. Required 3 stitches.

Employee cut his right index finger while placing a bag on the four spout bag packer. While initially believed to require only first aid the day of the incident, to ensure no infection occurred, a physician prescribed antibiotic the following day.

Employee was assisting in the removal of a steel plate from the floor beneath the crusher. The plate was dropped, striking the employee on the right leg.

Struck by powered moving object

Employee was preparing pallets for shipment in the #1 dock area. The forklift operator was moving pallets in the area. As the forklift operator was backing up, the forklift struck the effected employee in the left leg and ankle, resulting in a non-dislocated hairline fracture to the left tibia. Employee was treated by a doctor and returned to work.

Employee slipped on steps, resulting in a bruised elbow and back pain.

Employee received second degree burns to his left hand and wrist from an arc flash while pulling fuses on a submain feeder. Employee was seen at the emergency room.

Employee was standing on the front of a CAT 950 loader working on a clamshell feeder and slipped, falling approximately 6 feet. He fractured his knee and had superficial cuts on his forehead

The maintenance department was repairing a compressor located in the Mill Building. The crew had positioned the hub to install in the housing with a V45 CAT lift truck. The foreman was prying against the hub for better positioning when the fork assembly came off and fell across his hand.

EMPLOYEE WAS PULLING AT #1 BAGGER. WHILE DOING A LOAD THE SEAT HE WAS SITTING IN BROKE WITH HIM CAUSING HIM TO FALL. HE GRABBED A BAR ATTACHED TO THE BAGGER TO PULL HIMSELF ONTO THE MACHINE TO KEEP FROM FALLING INTO THE AREA BELOW.

EE WAS PACKING OFF BAGS AT #1 BAGGER MACHINE. LIFTED A BAG--TWISTED HIS BACK TO RELEASE THE BAG ON THE PALLET AND PULLED A MUSCLE IN HIS BACK.

Struck by falling object

EE, OUR MECHANIC, WAS CHANGING THE OIL IN A VEHICLE WHEN A PIECE OF RUSTY METAL DROPPED IN EYE. EE WASHED EYE OUT AND CONTINUED WORKING. THIS WAS ON FRIDAY. THE WEEKEND CAME AND HIS RIGHT EYE BOTHERED HIM. MONDAY MORNING WE SENT HIM TO THE EYE DOCTOR.
