Showing all 10The truck operator was driving from the material extraction point to the crushing plant when apparently lost the vehicle control and impacted the berm and as consequence overturned.
The miner did not notice that a pick-up truck was driving in reverse and was hit by the side of the truck. The driver of the truck stopped immediately once he notice the miner was in the ground. The truck was subject to inspection by MSHA personnel September 4,2014 and there was no deficiencies.
The employee was operating bobcat which was unloading the bucket and the machine overturned.
Driving the truck and the seat system failed causing injury to the employees back.

WHILE EE WAS WORKING ON A CONCRETE FOUNDATION BENEATH THE SIDE OF A CONVEYOR, THERE WAS A POWER FAILURE. WHEN THE POWER CAME BACK, THE CONVEYOR STARTED TO OPERATE, MAKING SOME ROCKS FALL DOWN, HITTING EE ON HIS BACK.
WHILE THE MINER WAS DRIVING A CAT 769C (DUMP TRUCK) LOADED WITH STONES UP A HILL THE TRUCK STARTED TO LOSE POWER AND HE STOPPED, THEN THE THE DRIVER TRIED AGAIN TO CONTINUE, BUT THE ENGINE OF THE TRUCK STALLED. HE TRIED TO BACK IT UP AGAINST THE EMBANKMENT AND WHILE DOING SO, HE TURNED OVER TO THE SIDE.
EE WAS INSTALLING A LOUDSPEAKER TO HIS VEHICLE WHEN HE WAS DISMOUNTING FROM THE CAR, HE WAS HIT BY ANOTHER CAR BY HIS CO-WORKER. EE FELL TO THE GROUND ROLLED FURTHER DOWN THE ROAD AND THE CARS REAR TIRE RAN OVER HIS RIGHT LEG.
THE EE SHUTDOWN A CONVEYOR BELT AND DIRECTED OTHE EE TO START THE CONVEYOR AS SOON AS HE FINISHED TO CLEAR THE MAGNETO. HE WAS STILL ON THE CONVEYOR WHEN THE OTHER EE TURN ON THE CONVEYOR WITH OUT SIGNAL. THE EE IN CONCERN TO GET TRAP ON THECONVEYOR HITTING THE MAGNET WITH HIS BACK.
