While operating a loader underground EE stated they hurt their lower back due to the uneven ground in the area where they were loading material. The EE was taken to the clinic where they were diagnosed with a lumbosacral strain in their lower back.

While EE was cleaning material off a catwalk a rock from an overhead conveyor fell and struck them in the back of the head as EE was bending over causing a laceration. The EE received staples at a local clinic to close the wound.

Employee was operating production loader (Catapillar 988G) digging shot rock in the pit as per usual when the bucket struck a ledge (known to the operator) jarring both machine and operator. This jarring lead to his complaint of neck pain, an appointment with the Company doctor & light duty for at least (2) two weeks. EE has complained of neck/back problems in the past.

Accident was reported on 3/23/07 as a strain to lower back will driving haul truck. Employee continued to work his regular job hauling mud until 4/3/07 when he returned to work that morning at 8:00am with a doctor restrictions until the 12th of April.

Employee was helping a contractor load and move out their welding machine on the contractor's forklift.The welder was not centered very well on the forks, and when it was picked up approx. 15" it started to fall. He stepped forward to help steady the welder and at the same time the forklift operator lowered the forks, setting the fork on his foot.

Engineer moving railcars to a particular track...the switch thrown to wrong track...cars already on it...cars pushed by the locomotive struck parked cars...engineer reported soreness...didn't request doctor until 08/07...diagnostics followed...08/25 engineer has Herniated Disc...treatment to be determined... no missed work days...continues as a locomotive engineer daily.

EE THOUGHT THE BELT HE WAS WORKING ON WAS 1-BC-3 & LOCKED IT OUT. THE BELT WAS ACTUALLY 3-BC-1. HE WAS IN THE PROCESS OF PUTTING A CLAMP ON THE SIDE OF THE BELT. THE BELT INADVERTENTLY STARTED PULLING HIS ARM INTO THE IDLER ROLLER & ALSO WEDGED HIS LEG BETWEEN THE HANDRAIL & THE BELT. NO LOST TIME, RESTRICTED WORK DUTY & PHYSICAL THERAPY ONLY.

OPERATOR WAS DRIVING AT AN UNSAFE SPEED. HE RAN INTO A BERM BUILD-UP AT THE EDGE OF THE ROAD, CAUSING HAULER TO FLY OVER BERM AND THROW HIM AROUND IN CAB, HITTING HIS HEAD. HE WAS WEARING HIS SEAT BELT.
