Showing all 9Employee was driving the service truck to the "boneyard". Employee drove over a small 6" dip in the road. As employee drove over the rough terrain employee felt soreness in the neck.
Employee was training another employee on the water truck. While approaching a curve the water truck seemed to be going too fast. The water truck flipped over causing minor injury to the passenger.
Employee complained of pain in right shoulder, neck and back from driving haul truck.
Employee was conducting a workplace examination on the primary conveyor belt and knocked off some built up mud on a troughing idler with a hammer. The hammer and employees left arm was pulled between the troughing idler and conveyor belt causing pain to left wrist. Got on a hopper, dumped load, slowly pulled off hopper, put on high gear. Thinking he was in low gear came off hopper too fast.
