Employee experienced shoulder pain while opening and closing a window in the office. Employee was sitting down while opening the window, overextending shoulders. The pain progressed throughout the week. Employee requested to go to the doctor on 12/18/2017.

While installing tracks on a John Deere 750 Excavator, the 20' chain that was hooked from the bucket to the tracks to pull the track onto the track frame broke, striking the employee that was standing approximately five feet from the left track frame on the left forearm, hand and forehead. Employee was examined by physician and released to restricted duty.

Employee was repairing (pit) water pump, when he stepped away from pump to wave over the trackhoe operator. The employee fell, causing pain and swelling on right knee. He was placed on light duty, will follow-up with Doctor on 11-14-05.
EE was installing repaired lift cylinder on a 750 John Deere trackhoe he positioned a ladder against the boom of the excavator, as the cylinder was lifted up and close enough for him to reach, the ladder started to slide along the boom of the excavator. He then lost his balance, fell & struck the rear bumper of the service truck resulting in pain to left shoulder & neck.
EE WAS INSTALLING REPAIRED LIFT CYLINDER ON A 750 JOHN DEERE TRACKHOE HE POSITIONED A LADDER AGAINST THE BOOM OF THE EXCAVATOR AS THE CYLINDER WAS LIFTED UP AND CLOSE ENOUGH FOR HIM TO REACH THE LADDER STARTED TO SLIDE ALONG THE BOOM OF THE EXCAVATOR HE THEN LOST HIS BALANCE AND FELL AND STRUCK THE REAR BUMPER OF SERVICE TRK RESULTING IN PAIN TO LEFT SHOULDER
