Over-exertion in pulling or pushing objects

Employee was pulling on a pry bar to remove the V-belts for the secondary shaker when releasing the pressure on the pry bar ee felt back tighten up.

Employee was replacing the rear tire on a service truck. Employee was trying to lift the old tire into another truck when the employee felt soreness in lower back.

Employee was replacing rollers on a conveyor belt. He was trying to force the roller frame between the belt, he lost his grip and smashed his left index finger between the roller frame and conveyor frame.

Struck by falling object

Employee was standing under overhead hopper when a 2.5 inch rock fell and struck on left shoulder, employee was examined by physician and was release to regular duty, employee followed up with physician on 3/28/13 and was put on restricted duty due to soreness until 4/11/13 follow up visit.

Employee complained of pain in right shoulder, neck and back from driving haul truck.

Employee was conducting a workplace examination on the primary conveyor belt and knocked off some built up mud on a troughing idler with a hammer. The hammer and employees left arm was pulled between the troughing idler and conveyor belt causing pain to left wrist. Employee was examined by physician and released to restricted duty and is to follow-up with doctor on 2-8-12.

Employee was removing wiper blade on primary shaker, while loosening bolts he felt pain in his left shoulder and neck, he was examined by a physician and was released to work with restrictions and will follow up with doctor on 4-28-2011.

Employee was replacing cutting edges on a 980H Front End Loader, the employee pushed up on a bolt that slipped out of the cut out, when another employee engaged the impact wrench cutting employee's right palm, employee received six stitches and was returned to work on restricted duty, employee will follow up with doctor on 3-17-2011.

Employee was climbing on the side of a Cat 750 Excavator to remove fuel hose. His foot slipped on bottom step an he felt pain between his right elbow and shoulder The employee did not request medical care at this time. Employee requested to see a doctor on 10/17/06 and was given restricted duty and returned to work.

Employee was hooking welding trailer to pick-up and was not lined up with trailer and tried to pull trailer to align with pick-up and strained his back. He was seen by a doctor on 5/3/06 and was told he strained his back. He was returned to work with restrictions and wil follow up with doctor on 5/12/06.

On 3/25/06, EE told his supervisor that his back was aching. On 3/27/06, EE wanted to see a Dr. He stated that his back had been hurting for a couple of weeks and believed pain was caused from a previous back injury several years ago. The Dr. asked EE what he had done and EE said his normal job duties, and on Saturday he had removed a pulley from a motor shaft.

Employee hurt right shoulder while adjusting tail pulley on feeder belt. He reported injury to plant manager but did not want to see a doctor at this time. On April 4, 2006 he requested to see a doctor and later was referred to an orthopedic doctor and is waiting to be scheduled for minor corrective surgery.

Got on a hopper, dumped load, slowly pulled off hopper, put on high gear. Thinking he was in low gear came off hopper too fast.

EMPLOYEE WAS HELPING LOAD STEPS INTO BACKHOE BUCKET. THE STEPS SLID IN THE BUCKET, SCRAPING EMPLOYEE ON THE LEFT THIGH CAUSING A BRUISE. EMPLOYEE WENT TO SEE THE DOCTOR AND WAS RETURNED TO LIGHT DUTY.
