The EE while cleaning drum pulley with rebar while operating. The rebar was caught by the drum pulley injuring index finger.

EE was cleaning between conveyor 20 and conveyor 7 area. A stone fell (from operating conveyor approx. 6" above EE's head), hitting EE on back of head, right side. EE was unconscious approx. 25 mins.; regained consciousness prior to being taken to hospital. EE was wearing hard hat at time of accident. Diagnosis: concussion.

THE EMPLOYEE WAS WALKING UNDER THE CONVEYOR WHEN A 3 TO 4 INCH STONE APPROX FELL ON HIS HEAD. THE EMPLOYEE HAD SECURITY HELMET BUT DIE TO THE IMPACT AND SIZE OF BOULDER EMPLOYEE SUFFERED A CUT TO HIS HEAD AND DIZZYNESS.
