EE was unconscious approx. 25 mins.; regained consciousness prior to being taken to hospital. EE was wearing hard hat at time of accident. Diagnosis: concussion.

THE EE WAS WORKING ON THE MAINTENANCE OF A CONVEYOR BELT, HE DIDN'T STOP THE MACHINE TO CLEAN IT, THEN THE MACHINE HURT A FINGER OF THIS EE.
