EE was running the loader , and when they were traveling from one end of the pit to the other to load trucks they hit a bump and the seat in the loader bottomed out.

Showing all 11While operating a loader, EE was cleaning the road with the bucket flat going downhill. The loader dug into the floor causing the loader to stop abruptly. Employee hit EE's head on the front window of the loader.
A Komatsu 605HD rolled over backwards at the lower crushing material dump site, landing upside down. The wall was approximately 25 feet high. Employee was wearing a seatbelt at the time of the roll over.
Employee was driving a haul truck and was being loaded when that caused EE to be jarred in the cab of the truck causing muscle spasms in EE's back. This occurred on 11/22, went to the Dr. on 11/23 and treated conservatively until yesterday. The Dr. hasn't seen any improvement so Dr. ordered Physical Therapy and prescriptions making the recordable date 11/29/22.
Employee covered the loader shift for 3 hours. When they exited the loader around 12, they had some back pain/discomfort. Thinking it was normal back pain that they had felt in the past, they assumed it would feel better in the morning. When they arrived at work today it was clear that they had strained their back bad enough that it was hard for them to stand up.
Employee Rolled a Skid-steer, was released to full duty. Continued having pain, became recordable from MRI results on October 26, 2018 showed Tear in L. shoulder tendon.
Employee was guiding belly dumps into dump. EE was standing behind a 4ft wing wall. As the truck dumped EE's main, EE became stuck on the material EE was off loading, and spun EE's rear dual tires & threw a 2 inch rock, approx. 20 feet over the wall, and struck our employee in the lip chipping EE's tooth.
FIRE EXTINGUISHER HAD WENT OFF INSIDE CAB OF LOADER, OTHER EMPLOYEE CLEANED OUT THE DEBRIS, HOWEVER, WHEN INJURED EE GOT IN LOADER TO START HIS SHIFT, HE TURNED ON THE HEATER & DEBRIS/CHEMICAL BLEW OUT INTO HIS EYE.
DIRT & ROCKS CAUSED THE RETURN ROLLER TO STOP. EE WAS USING A METAL BAR TO BREAK UP THE DIRT & ROCKS. AS THE MATERIAL BEGAN TO BREAK UP, IT SET THE BELT IN MOTION. THIS CAUSED THE BAR TO BE PULLED INTO THE ROLLER. EE HAD A TIGHT GRIP ON THE BAR & HIS FOREARM WAS CAUGHT IN BETWEEN THE BELT & THE ROLLER. EE WAS ABLE TO PULL HIS ARM FREE FROM THE BELT BY PULLING ON BICEP.
EMPLOYEE WAS LUBRICATING A BELT ON THE FORKLIFT IN PREPERATION TO REMOVE THE BELT. ANOTHER EMPLOYEE OPERATING A BOBCAT WAS PULLING ON THE FORKLIFT, THE BOBCAT'S WHEELS BEGAN TO SPIN SO THE OPERATOR TURNED IT OFF. THIS CAUSED THE FORK OF THE LIFT TO DROP ONTO THE MECHANIC'S HAND.
EE WAS DRIVING THE LOADER TO RETRIEVE ROCK MATERIAL. DRIVING ON THE DESIGNATED HAUL ROAD WHEN THE HYDRAULIC TILT BLEED OUT, CAUSING THE BUCKET TO TIP DOWN AND DIG INTO THE GROUND. WHEN THIS OCCURED THE EE WAS THROWN FORWARD STRIKING HEAD, EE WAS WEARING SEAT BELT.
