EE was running the loader , and when they were traveling from one end of the pit to the other to load trucks they hit a bump and the seat in the loader bottomed out.

EE was pulling the guard from the cone to replace the V-belts. EE hooked the cone above EE's head in the belt frame. There are two holes to hook the cone to, the chain on the cone would not release so EE jerked on it a couple times and it came out of the hole and struck EE in the face. This caused a laceration of the upper lip that required stitches.

Heber crew was changing screens. Employee and another employee were putting the strap iron to hold the potato belting on the back of the screen. Employee was holding the bolt laying down on the bottom deck with their legs sticking out when the piece of metal fell and struck employee's leg.

Employee was removing skid steer bucket and buckets pins would not release. He and another employee curled the bucket to access the bucket pins to tap them in all the way. While working on pins, the bucket became unhooked and fell two feet onto employee's foot above his steel toe.

EMPLOYEE WAS HEATING A CONVEYOR ROLLER WITH A TORCH. THE TORCH CAUGHT ON CONVEYOR BELTING AND WAS PULLED FROM EMPLOYEES HAND LACERATING HIS FINGER

Cutting flashing belt for under jaw conveyor/hopper with his right hand when the knife slipped and cut 2 fingers on his left hand.

EE WAS OPERATING BOBCAT UP A HIL, HE ACCELERATED TOO FAST, CAUSING BOBCAT TO LOSE TRACTION AND ROLL OVER.
