Operator of loader was taking slab of stone off conveyor. Slab shifted on forks, fell off of forks, hit the guard rail. Guard rail fell off and hit operator's foot.

While operator was performing a splitting operation, a slab of stone fell and caught the operator's left arm causing an abrasion.

While moving a pallet of stone slabs ,operator extended forks on the Teledyne fork lift to set pallet on floor.In doing so the machine tilted forward and operator hit his head on the roll cage causing a laceration above left eye.

Driving the loader during the shift. EE said he was not experiencing any pain in the morning at the beginning of the shift. He started experiencing some pain by 9:30 a.m., but did not report this to the supervisor. By 11:15 a.m. he informed the supervisor that his back was hurting and he needed to see a doctor.
