The laborer was trying to secure a large rock inside the bucket of a skid steer when the operator of the skid steer accidentally dropped the bucket on the laborers foot. The extent of the injury is not known at this time.

Miner was standing under forklift mast that was being supported by another forklift and a sling. The sling felt off the forks letting the mast fall striking the miner and landing on his left foot.

While operating a loader, the employee was dumping material into a grizzly. The tilt control on the loader stuck allowing the bucket to roll forward hitting the bucket stops. This jarred the loader causing an injury to the employees lower back. Lower lumbar strain.

Employee was operating loader controls for another mechanic who was changing bucket pins. The cab was getting warm, so EE turned on the A/C. He then stuck his hand into the vent cavity (the vent louver was off & sitting on the dash in front of EE) to feel the air temperature. His left index & middle fingers came in contact with the A/C fan blower wheel.

Employee was operating a Euclid Haul Truck transporting material from the pit to the crusher when towards the end of his shift hit a pot hole and felt a pop in his lower back. He was feeling a sharp pain run down his back side and down his legs.

Ee was operating the 777 & hit a pot hole and the seat suspension was not operating properly and hurt his back. EE told us about this on the 30th of Oct that he hurt his back not on the 18th.
