Miner was operating an excavator (Cat 385) when they experienced a seizure. Coworker in a truck (Cat 745C) arrived to load and notice the excavator boom extended instead of in loading position and saw the operator in the cab leaning against the window. Coworker found the excavator operator to be unconscious and turned off the excavator. Coworker called supervisor and 911.

Employee complained of nausea and asked to go home. Other employees recognized symptoms consistent with a heart attack, and called 911. It was later determined that the employee did suffer a heart attack.

Struck by rolling or sliding object

EE was cutting a piece of 6"" channel steel at the steel rack behind the shop. He lifted the channel up and shored it with another smaller piece of steel so he could cut with the welding torch, he turned around to grab the torch, at that time some steel pipe slid off the rack and struck his right foot.

Contact with hot objects or substances

EE was heating a part to remove a broken steering ball stud in a cat 775D haul truck. Some hot grease spat back at him and struck his right forearm and caused him to srop the hot torch. The torch flame flashed by his side and burned his abdomen.

Struck against a moving object

Employee was operating loader controls for another mechanic who was changing bucket pins. The cab was getting warm, so EE turned on the A/C. He then stuck his hand into the vent cavity (the vent louver was off & sitting on the dash in front of EE) to feel the air temperature. His left index & middle fingers came in contact with the A/C fan blower wheel.

EMPLOYEE WALKING ON DECK REBAR APPROXIMATELY 12"" HIGH LOST HIS FOOTING CAUSING HIM TO SPRAIN/STRAIN HIS RIGHT ANKLE. EMPLOYEE DID NOT INFORM EMPLOYER UNTIL 11/4/05.

Struck by flying object

WHILE EE WAS USING A CHOP SAW TO CUT A PIECE OF METAL, THE CHOP SAW DISC BROKE. A PIECE OF THE DISC FLEW AND HIT HIS FINGER, MAKING A SMALL CUT ON HIS RIGHT INDEX FINGER.

Fall from machine

While stepping from machine to a ladder, pant leg got caught on the battery box. This caused him to fall, landing on his left side.

Struck by falling object

WHILE SHOP MECHANIC WAS WORKING ON MOTOR GRADER ALTERNATOR, HIS ELBOW BUMPED THE HOOD SAFETY LATCH BRINGING THE HOOD DOWN ON TOP OF HIS HEAD. THIS RESULTED IN ABOUT A ONE INCH SUPERFICIAL LACE RATION ON TOP OF HIS HEAD.

WHILE CHANGING A SCREEN ON OUR PORTABLE PLANT, EE WAS SWINGING A HAMMER TO BREAK FREE THE OLD SCREEN. HE MISSED THE TARGET AREA AND HIT HIS LEFT THUMB. THE CONTRIBUTING CAUSE OF THIS INJURY WA S HIS HAND IN THE PATH OF THE IMPACT AREA.

LIFTING A STEEL PLATE WITH A SLING ATTACHED TO ELECTRIC HOIST. SLING SLIPPED OUT OF HOIST HOOK AND THE STEEL PLATE FELL OUT OF SLING HITTING LEFT FOOT EMPLOYEE.
