Showing all 19The employee was holding the shaft with the crane. When EE raised the hoist, it moved faster than anticipated, causing the synthetic lifting strap to fail. This caused the hook block to jolt and become unattached to the rope cable, then falling to the floor. The hook block then struck the sync motor and then struck the employee's right middle and right ring finger.
Associate was assisting in track maintenance near the switch stand, when the backhoe pressed against the switch point, causing the switch handle to swing in the opposite direction striking the associate in the side of EE's face and ear.

A miner driving a UTV 4WD personnel carrier struck a contractor that was walking in the crosswalk between the Patras building and office buildings. The contractor felt pain from left elbow to fingers specifically left ring and pinky fingers, they also felt pain on left thigh as well as the back of the head.

Employee suffered a hip contusion when a utility vehicle failed to stop on time and struck the rear of a company truck where employee was standing. Employee was knocked to the ground injuring hip.
Employee was transporting manlift back to its location when he noticed an incoming vehicle, as he moved to allow access, the manlift front wheel fell into a pothole causing employee to lose footing and fall against the basket. As a result of the fall, he sustained lacerations on his elbow and forearm.
Employee was commuting from primary crusher to plant when he suffered an asthma attack, lost control of his vehicle, and collided with an electrical pole located in median of the the access road. As result, he sustained a contusion in chest wall and closed fracture of rib.

SUNNY DAY, RAMP IN GOOD CONDITION. WHILE EE PROCEEDED UP RAMP, THE LEFT STEERING CYLINDER TIE ROD BROKE, PULLING THE TRUCK TO THE LEFT. HE THEN TRIED STEERING TOWARDS THE RIGHT BUT THE TRUCK DIDN'T RESPOND AND ROLLED OVER.
