EE WAS TRAVELING OUT OF THE MINE FOREIGN MATERIAL FELL FROM THE TOP HITTING THEM IN THE FACE AND EYES

EE was riding on a 2 man rubber tired Johnson and ran over some gob in the roadway causing ee to jam head into a roof strap. EE felt pain in neck and shoulders and into both hands.

While tramming a loaded shuttle car away from the face the car hit a bump and roofed out causing the operators head to strike the canopy.

The continuous miner operator was struck in the back by a shuttle car while checking his sight in the belt entry.

Hood wouldn't slide over engine compartment. Picked up a binder to use as a pry bar. Put binder under hood, pushed down and right middle finger came into contact with rotating fan blades lacerating and breaking right middle finger.

EE WAS ATTEMPTING TO CONNECT LOCOMOTIVE TO WATER CAR. EE DID NOT POSITION ENTIRE BODY IN THE OPERATORS COMPARTMENT, KNEELING IN THE COMPARTMENT LEAVING FEET EXPOSED. EE TRAMMED THE LOCO FORWARD AND LEFT FOOT WAS PINCHED BETWEEN THE LOCO AND RAILS STACKED ALONG RIB. CAUSE: INPROPER BODY POSITIONING
