Employees left ankle/heel was hit by the steer tires of a forklift. The injury occurred on 2/29/08 however; it did not become recordable until 3/21/08, when prescription medicaiton was needed.

EE removed guard, pushed on vee belt to check belt, causing belt to grab and start. This pulled EE's finger into pully and cut off the right index finger at the base of nail.

OPERATING THE TRACK LOADER OVER A BERM 3'HIGH INTO A DRAINAGE DITCH WHEN LOADER WENT OVER BERM. THE FRONT END HAD A SUDDEN DROP WHICH THREW THE OPERATOR FORWARD INTO THE CAB. OPERATOR HIT FOREHEAD ON THE REAR VIEW MIRROR MOUNTED IN THE CAB. THE ONLY DAMAGE WAS TO THE MIRROR, WHICH WAS BROKEN. EE WAS WEARING HIS SEATBELT.
