Employee was taking product line loose to hook back to silo when it had got stuck. Employee was bent over using his hands and applying pressure when it cam apart flipping up hitting employee in the mouth.

Struck against stationary object

Two employees where installing the master link of the track on a PC400 track hoe. The employee was using a hammer to drive the pin through the track and his left hand hit the idler cover located in the middle of the track. The employee was taken to Sister of Mercy Urgent Care where it was determined that he had a closed fracture of his left little finger.

Injured had dsended the steps of the haul truck, turned and wedding ring caught on step grading causing flesh to be torn under ring.

EE removed guard, pushed on vee belt to check belt, causing belt to grab and start. This pulled EE's finger into pully and cut off the right index finger at the base of nail.

EE IS AN EXPERIENCED WELDER AS HE'S HAD HIS OWN WELDING BUSINESS. HE SIMPLY DIDN'T FOLLOW COMPANY POLICY OF WEARING P.P.E. WHICH IS PROVIDED.
