Employee was returning to the pit from primary. As EE neared the bottom of the long ramp, the auto retarder engaged which caused EE's shoulder belt to lock up and slip up EE's neck. Strain to unspecified parts of the neck.
Employee was utilizing a water truck on a haul road to wash out return rollers on conveyor and while driving lost control of the truck resulting in a vehicle accident.
Employee was cleaning around the plant in a skid steer working around the surge pile tunnel at the quarry, the weather was wet and the employee was dumping a bucket load of wet sand on to a spoil pile near the tunnel. As the employee dumped the load of material it splashed back and the material splashed on the employee's face running down behind their safety glasses.
Inattention to surroundings by the operator. In addition, task fixation (focusing on the stacker being down) was a contributing factor.
Teen age kids were traveling S on Bolander Rd. went off the R side of road then off the L side of road and hit parked RR cars sitting on the side track.
Loading truck
EE was cleaning up after a shot and he didn't have his buckel all the way down on the ground, He ran over a rock. That made his upper body twist and his head hit the center post in the cab. He cut his head on the left side and had to get 2 staples in his head. I took him to hospital. He returned to work with out restrictions.
EE drove his truck down to the lower bench and stopped his truck 100' behind and to the left of thr 992G Cat loader, in a blind spot wher the loader operator (injured) could not see him. Not knowing truck was in area injured cont. to back drag floor for 2nd operation. Truck driver seen loader backing up darted truck forward in path of loader hitting tailgate of truck bed.
While backing an R65 Euclid truck up a ramp to dump fill material, the left rear tires went into a low spot causing the truck to dip. Employee said this caused him to be thrown against the door handle of the truck. We do not know if he was wearing a seat belt. This injury did not become reportable until he had surgery on Sept. 29, 2006.
MILL OPERATOR TRIED TO REMOVE A LODGED ROCK FROM A RUBBER IMPACT TROUGHING ROLLER, WHICH PREVENTED THE ROLLER FROM TURNING. THIS RESULTED IN A BRUISED HAND.
EE AND COWORKER WERE TRANSPORTING AND RELEASING A NON-RUNNING EUCLID HAUL TRUCK ON SITE. EE RELEASED HIS SIDE A LITTLE BEFORE COWORKER COMPLETED RELEASING THE RATCHET BINDER AND CHAIN ON HIS S IDE TO ALLOW THE AXLE AND WHEEL ASSEMBLY TO LOWER TO THE BLOCKING BENEATH THE RIGHT FRON TIRE. WHEN IT WAS FULLY RELEASED, THE TRUCK MOVED FORWARD AND TO THE RT. WHEN IT MOVED IT CAUGHT EE'S
EE ALLEGES TENDERNESS TO HIS BACK FROM BEING JOSTLED IN THE HAUL TRUCK WHILE SHOT ROCK WAS BEINGLOADED WE ARE CURRENTLY EVAULATING THE CLAIM TO DETERMINE IF THE ALLEGED INJURY IS WORK RELATED
EE WAS WORKING AS A PRIMARY/FACE LOADER OPERATOR. WHEN ENTERING THE MUCK PILE, HE HIT A LARGE ROCK AND JARRED HIS BACK. THIS IMPACT CAUSED LOWER BACK PAIN AND MUSCLE SPASIMS. THIS IS AN AGGRIV ATION OF A PREVIOUS INJURY.
