Showing all 12Maintenance worker was moving man lift from north side of plant to the south side of the plant. While traveling down a ramp in the lift a hydraulic hose failed causing the operator to lose control of the lift. The man lift fell over resulting in the operator suffering a laceration to the left shin and a strained shoulder.
An employee was standing under the rail load out conveyor. A frozen piece of material fell from the frame of the conveyor and struck the employee on the hard hat knocking employee to the ground. The employee suffered serious neck and back injuries.
Employee was driving Cat 740 haul truck and stated back started to hurt. Employee was diagnosed with a sprain/strained back.
Miner operating a haul truck hit a bump in the pit area and felt a pop in EE's neck. Miner suffered a strain/sprain to the right side of EE's neck. The haul road from plant to pit was inspected and no significant bump (uneven ground) was found.
EE did his vehicle inspection went to climb in the loader when he reached up to grab the hand rail he felt a pop in right shoulder and a slight pain about and hour later it was slightly burning and sore. He reported the incident and refused medical treatment. He treated under his medical insurance until he needed an MRI.
Employee was walking under the diester screen, when a rock fell off the belt and landed on his left foot, resulting in a fracture.
EE was operating a loader on 2/23/09 when she hit several potholes causing strain to her neck. On 2/27/09 while operating a loader, again she hit several potholes causing strain to her neck. EE did not report injury to supervisor until 3/4/09.
EE WAS STANDING ON CATWALK, WASHING DOWN CONVEYOR WHEN A PIECE OF METAL LATTICE BROKE OFF FRAME, LODGING BETWEEN BELT AND ROLLER. THE PLANT OPERATOR SHUT DOWN CONVEYOR. THE LOADED BELT BEGAN TO ROLL BACKWARD, CAUSING EE'S HAND TO BE PINCHED BETWEEN PIECE OF METAL AND FRAME.
EE WAS OPERATING THE EUCLID WHEN THE REAR AXLE HOUSING BROKE. THE UNIT DROPPED TO THE LEFT REAR SIDE CAUSING EE TO HIT HIS BACK AGAINST THE DOOR ON UNIT.
EE ALLEGES THAT WHILE DRIVING HAUL TRUCK, HE HIT A BUMP INT HE ROAD, CAUSING LOWER BACK STRAIN.
EMPLOYEE WAS STANDING NEAR SHAKER ATTEMPTING TO HOOK UP A WATER HOSE WHEN A ROCK FELL OFF OF CONVEYER AND HIT EMPLOYEE IN THE MID BACK AREA.
