Employees onsite smelled smoke at the beginning of the shift. Production supervisor went to see where smoke was coming from and observed a fire adjacent to mine property. At the end of the shift around 6:00 pm the fire had spread to mine property, so production supervisor call the fire department. No employee injuries and no mine asset damage.

Employee was wearing improper cut level of gloves when hammering metal on metal. Metal piece separated, cut through employee's glove and into the skin between two knuckles.
Employee was performing lawn care duties and began to exhibit allergic reaction to a plant and/or vegetation the employee encountered. Employee went to seek medical care and was diagnosed with contact dermatitis.
Dozer operator exited the dozer cab and pinched left pinky finger in the door as EE closed the cab door.
Maintenance employee was removing a tooth from the 992 loader. EE was striking the tooth with a hammer when a shard of the tooth flew off and struck EE on the inner thigh of EE's right leg resulting in the shard being embedded in EE's groin area.
Employee was bit by an insect while using a porta potty and experienced an allergic reaction from the insect bite.
A plant operator was removing a roller frame from CB-2B conveyor tail end. While pulling the roller out EE's grip slipped and resulted in dropping the roller and it landed on EE's foot approximately one inch above protective steel toe.
Water truck operator was cleaning the area around the secondary crusher with a water hose when a rock fell from above and struck the employees hard hat. The employee began to experience a headache and nausea. The employee was diagnosed with a concussion.
On 2/13/22 at approximately 9:45am the plant supervisor was notified of a fire at the site. 911 was immediately notified and the fire department and police department responded. A Caterpillar 980M was on fire. The fire was initially reported near the cab of the loader. The fire was extinguished.
Haul truck driver stated: drove over a pothole in the road then began experiencing lower back pain and numbness in left leg.
Admin asst entered shop to clock in for the day. As EE entered the building EE slipped on silt and mud that was on the ground. EE injured right hip. Injury initially treated with first aid measures. Employee received medical treatment on 08/09/21.
Maintenance worker was moving man lift from north side of plant to the south side of the plant. While traveling down a ramp in the lift a hydraulic hose failed causing the operator to lose control of the lift. The man lift fell over resulting in the operator suffering a laceration to the left shin and a strained shoulder.
An employee was in a man lift assisting with installing a new conveyor belt. As the new belt was being pulled onto the conveyor it began to slip off and the employee reached out to grab the belt to keep it from falling to the ground. The employee injured left bicep.
An employee was standing under the rail load out conveyor. A frozen piece of material fell from the frame of the conveyor and struck the employee on the hard hat knocking employee to the ground. The employee suffered serious neck and back injuries.
Plant operator walked into the office and told office workers they had a pain in shoulder. EE was going to get the shoulder checked out. Employee drove to local hospital. Medical staff told employee they were having a heart attack and rushed employee to hospital in the city.
Equipment operator exited D9 dozer and fell down to the track. Operator suffered laceration to right knee.
Electrician was performing electrical portion of installing a new rock hammer at the primary crusher. As the electrician was removing the motor starter of the old rock hammer an arc flash occurred. The electrician suffered 1st and 2nd degree burns to face and arm.
Employee was operating the water truck when the front left tire blew out. The window was down when the tire blew. The sound of the blow out caused pain in employee's left ear.
Employee was attempting to place a shackle over the frame of a log washer and smashed right pinky finger resulting in stitches.
A plant operator was changing the bearings on a field conveyor. EE was using a wrench to loosen a bolt. The wrench slipped off the bolt and the employee hit right elbow on the feeding trough. Injury was initially first aid. Management found out on 02/17/20 that employee was prescribed pain medication by a DR from this incident. Employee never got the prescription filled.
Plant operator was using a hand held grinder to cut the bearing housing off the tail pulley shaft. The grinder kicked back and came into contact with the employees left foreman. The employee suffered a laceration that required 16 sutures to close.
Two employees lifted a metal gear box to discard it in a metal waste bin. One of the employees got their right middle finger pinched between the gear box and waste bin. Employee suffered a laceration that required 3 sutures.
Employee was driving Cat 740 haul truck and stated back started to hurt. Employee was diagnosed with a sprain/strained back.
Production supervisor exited EE's truck to open a gate. EE brushed up against a bull nettle plant and suffered an allergic reaction. Pain and swelling in EE's left knee.
Plant manager was assisting employees with driving in a bushing on an excavator bucket. The plant manager was using a 20 lb sledge hammer to drive in the bushing. The next day the plant manager felt pain in the back and hands. Their hands also started to swell.
Employee was adjusting the forks on a skytrack and pinched finger between the fork and mast of the skytrack.
The plant operator went to shut off the water valve that was left open overnight due to freezing weather. As EE approached the valve EE slipped on some ice that was covering the plate and fell hitting EE's knee on the corner of the plate that was embedded into the concrete slab causing a puncture below EE's right knee cap.
Dredge operator tripped on swing cable while stepping from dredge ladder to pontoon. Employee fell to the deck of the pontoon suffering a sprain/strain lower back.
Employee was helping with installing stairs on primary crusher. Employee was pulling on stairs to get them aligned so the stairs could be welded in place. While pulling on stairs employee felt a pop in the right shoulder. Employee suffered a sprain right shoulder.
Two miners were attempting to lift a conveyor belt scale that weighs approximately 60 pounds. One of the miners strained a muscle in lower back while lifting the scale.
Miner was reaching for wheel chalks with EE's gloves on. A bee on the wheel chalks stung miner through glove. Miner immediately started feeling dizzy and had difficulty breathing.
Employee tripped over rug lying on office floor.
Miner suffered a strain neck when ee was struck on the top of hard hat by a lump of dirt that fell from the top of an excavator bucket that was resting on the ground about to be greased. The injury was treated as first aid and after a week the injury did not get better. Miner sought medical treatment on 10/19/2016 and was placed on restricted duty starting 10/20/2016.
Miner operating a haul truck hit a bump in the pit area and felt a pop in EE's neck. Miner suffered a strain/sprain to the right side of EE's neck. The haul road from plant to pit was inspected and no significant bump (uneven ground) was found.
Employee went to sit in a chair . The chair malfunction and the employee and chair fell backwards resulting in a strain to the right elbow
Employee was washing slab under hopper with water hose when the clamp failed and the nipple came off the hose cutting the left hand between thumb and index finger. 4 stitches and tetanas shot.
Employees were moving a 55 gallon drum of oil from containment area. The barrel slip from the ledge of the area causing the dolly to strike EE in left hip and straining his lower back. No treatment was sought until July 7/8/2015. EE was evaluated 7/24/2015 MRI results 08/04/15 prescription meds and physical therapy changed first aid to medical.
Employees were cleaning up around the outside of shop lifted a weart plate to clean off slipped out of the hand of the employee and hit the top of employee's right foot
while lifting a screen in the air with a crane a spring came off falling to the ground bounced off the catwalk an struck employee in the right leg
The third party driver collapsed while returning to his truck from getting his weight ticket. The employees at the site performed emergency procedures called EMS and performed CPR until medical help arrived and the driver was transported to hospital where he died days later of as a result of a heart attack.
While exiting the load the employee stepped in the mud and his foot got stuck. The employee strained his left knee trying to get his foot out of the mud.
Employee stated he felt a pull and tightness in his left knee while climbing the stairs helping carry a metal strap. Did not want treatment until 20 days later, left knee sprain strain
Employee was removing a down spout from the chute under sand classifier when it slipped and pinched the tip of his left ring finger between the spout and the edge of the chute resulting in a laceration requiring stitches. Employee was wearing all PPE including gloves.
Driller was adding addition to auger. When the auger stop rotation the helper reached for the pin as the driller reversed rotation and the head of the bolt pinched his thumb against the outer portion of the yoke. Helper had a fracture to right thumb.
EE did his vehicle inspection went to climb in the loader when he reached up to grab the hand rail he felt a pop in right shoulder and a slight pain about and hour later it was slightly burning and sore. He reported the incident and refused medical treatment. He treated under his medical insurance until he needed an MRI.
While performing maintenance on the conveyor EE crawled over conveyor belt from one catwalk to another to check bearing placement while crawling back to original catwalk he placed his foot on a piece of wood on the catwalk rolled his left ankle resulting in sprain/strain/contusion to the left foot and ankle.
Workers were using a crane and tag line to remove old conveyors when the structure holding the bottom section of conveyor fell striking the employee holding the tag line in the right shin. EE had laceration to right shin requiring 9 stitches
EE was attending annual refresher training stepped outside during break and was stung by a bee. EE allergic to bee stings
EE was adding 5 gallon bucket of antifreeze to the dozer and felt pain in back pain went away continued working woke up next day Sunday sore Treatment Monday muscle spasm and low back pain treated with prescription medication.
