Showing all 17Miner was stacking stone on a pallet when a loader passing by flipped a stone striking the miner in the left knee.
Miner was attempting to re-attach a blower guard in the loader. EE's finger got too close to the blower fan and was cut by the fan blade.
The miner was driving a loader with the bucket very close to the ground. The bucket hit a stone and stopped abruptly. Although EE was wearing seatbelt, EE's upper body lurched forward and EE's head struck a switch on the ROPS pillar. EE was cut above right eye and required stitches.
Employee was operating a loader on a bumpy surface. He experienced pain in his lower back. Went to the Dr. Was given meds. to reduce swelling and relax muscles.
Worker was operating a loader removing stone from a ledge. He felt his back tighten and soon it became painful. This injury was NOT reported to the company. The worker went to the doctor and was put on restricted duty.
Building on mine property was over-exposed to carbon monoxide gas from an improperly tuned forklift. Worker was taken for medical treatment. No work time was lost.
Employee reported to me that he has been having pain in his right shoulder for some time; he feels that it is a work-related repetitive motion injury.
He twisted while operating a forklift and felt back pain.
Stone stuck in the conveyor belt. EE pulled up metal trip bar to remove stone. Trip bar dropped on his hand when he took the stone out & caused a fracture to left middle finger.
Was a passenger in a pick up truck and the truck hit a bump and the employee's knee hit the dashboard in the truck.
EE WAS ON THE GROUND WORKING, WHEN A STONE WAS THROWN UP OR OFF OF THE LOADER TIRE. THE INJURY WAS TO HIS RIGHT LEG.
HIS LOWER RIGHT BACK STARTED TO HURT. HE WAS DOING NOTHING DIFFERENT THAN EVERY DAY THINGS. HE WAS IN & OUT OF HIS LOADER HE SAID. IT JUST JARRED & STARTED TO HURT.
WAS DRIVING THE FORKLIFT & A STONE FLEW UP & HIT HIM IN THE BACK OF THE HEAD.
HE SAID ALL OF A SUDDEN AFTER LUNCH HIS RIGHT TOP FOREARM HAD A LUMP ON IT. HE WAS DRIVING THE FORKLIFT WHEN HE NOTICED IT. HE SAID IT MUST BE FROM SHIFTING. HE WASN'T SURE.
EE WAS ON HIS WAY TO A TRADE SHOW IN MINNESOTA AND LOST CONTROL OF VECHILE HE WAS DRIVING AND TRUCK FLIPPED OVER 3X15.
EE WAS A PASSENGER IN A VEHCILE DRIVEN BY OTHER AND VECHILE FLIPPED OVER 3X15.
