Miner has worked entire career at this mine repairing heavy and light equipment. The repetitive motion after years of using tools to repair mining equipment has caused carpal tunnel symptoms in right and left wrists and hands.

Fall from machine

Miner was climbing down the ladder on a 35 ton haul truck. The ladder was broken and EE's foot slipped through the ladder while the ladder flipped up into the wheel well. EE fell backwards onto the ground 3 feet below. EE landed on EE's back with EE's leg stuck in the ladder. EE did not submit EE's injury report until 02/08/2022.

Miner was stacking a piece of stone on a pallet when a finger was pinched between two pieces of stone. Miner received a crushing injury to the tip of the little finger and the finger nail bed.

Absorption of radiations, caustics, toxic and noxious substances

Miner was filling the propane tank on EE's forklift from a bulk propane tank. As EE disconnected the filling hose, the pin in either the filling nozzle or the forklift propane tank stuck open allowing propane to escape under pressure. The propane hit the back of the miner's hands and caused freeze burns resulting in skin loss on the back of the right and left hands.

Miner was sizing a piece of stone using a hammer and chisel. EE swung the hammer very hard and lost control hitting the chisel with a glancing blow. EE's hand smashed into the stone cutting the right index finger and causing a fracture of the right index finger.

Miner was late for shift. Had just punched in and was walking to the job site and slipped on the wet pavement and fell. Miner broke fall with right hand and injured hand.

On 01/29/2012 the worker went to the Dr. claiming vertigo, headaches and blurred vision. He was given meds and restrictions. This incident is under investigation for possible workers compensation fraud.

His surgery was on 8/12/2011. He was off the day of the surgery. He returned to work on 8/15/2011 with meds but no restrictions. On 8/19/2011 he was seen by his Dr. and put on restrictions.
